摘　　要：目的:研究用动脉灌注化疗联合栓塞术治疗中晚期肺癌合并咯血的临床效果。方法:将上海交通大学医学院附属新华医院崇明分院收治的80例中晚期肺癌合并咯血患者随机分为A组(n=38)与B组(n=42)。对A组患者进行静脉化疗,对B组患者进行动脉灌注化疗联合栓塞术,然后比较两组患者的近期疗效、咯血症状的改善情况及其不良反应的发生率。结果:经治疗,B组患者的RR、DCR及其咯血症状的总改善率均高于A组患者,P <0.05。治疗后,B组患者Ⅰ级～Ⅲ级恶心呕吐、Ⅰ级～Ⅲ级白细胞减少和Ⅰ级～Ⅲ级血小板减少的发生率均低于A组患者,P <0.05。结论:用动脉灌注化疗联合栓塞术对中晚期肺癌合并咯血患者进行治疗可显著提高其近期疗效,改善其咯血的症状,且其恶心呕吐、白细胞减少和血小板减少等不良反应的发生率较低。objective:to study the clinical effect of arterial perfusion chemotherapy combined with embolization in the treatment of advanced lung cancer complicated with hemoptysis.Methods:80 patients with advanced lung cancer complicated with hemoptysis admitted to chongming branch of xinhua hospital affiliated to Shanghai jiaotong university were randomly divided into group A(n=38)and group B(n=42).Patients in group A were treated with intravenous chemotherapy,and patients in group B were treated with arterial perfusion chemotherapy combined with embolization,and then the short-term efficacy,improvement of hemoptysis symptoms and the incidence of adverse reactions in the two groups were compared.Results:after treatment,the total improvement rates of RR,DCR and hemoptysis in group B were all higher than those in group A,P<0.05.After treatment,B group of patients levelsⅠ~Ⅲnausea and vomiting,Ⅰ~Ⅲleukopenia levels andⅠ~Ⅲincidence of thrombocytopenia than A group of patients,P<0.05).Conclusion:arterial infusion chemotherapy combined with embolization for the treatment of patients with advanced lung cancer complicated with hemoptysis can significantly improve the short-term efficacy,improve the symptoms of hemoptysis,and the incidence of adverse reactions such as nausea and vomiting,leukopenia and thrombocytopenia is low.
