摘 要:目的 探讨食管癌放疗胸骨受照射剂量对骨髓抑制发生的影响。方法 选取2014年1月—2017年1月本院接诊的200例食管癌放疗患者作为研究对象,根据患者在接受放疗治疗中所表现的血象变化特征,将患者分为骨髓抑制组与无骨髓抑制组两组,每组各100例。对比分析两组患者的胸骨受照射剂量与骨髓抑制发生的关系。结果 骨髓抑制组的D mean值为(2905.03±110.57)(c Gy),高于无骨髓抑制组的(1938.21±91.02)(c Gy),差异具有统计学意义(P<0.05);骨髓抑制组的V20值、V30值与V40值分别为(63.56±4.59)%、(52.03±4.31)%、(25.16±2.92)%,无骨髓抑制组的V20值、V30值与V40值分别为(42.17±3.48)%、(23.98±3.02)%、(8.55±1.90)%,组间比较差异具有统计学意义(P<0.05)。结论 食管癌的放疗胸骨照射剂量与骨髓抑制的发生密切相关,且在食管癌放疗中应做好胸骨保护。Objective To investigate the effect of sternal irradiation dose on the bone marrow suppression in radiotherapy for esophageal cancer.Methods From January 2014 to January 2017,200 cases of patients with esophageal cancer received radiotherapy in our hospital were selected. According to the characteristics of blood changes in the treatment of radiotherapy, the patients were divided into bone marrow suppression group and nonbone marrow suppression group, 100 cases in each group. The relationship between sternal irradiation dose and bone marrow suppression in radiotherapy for esophageal cancer of the two groups was compared and analyzed. Results The D mean value of bone marrow suppression group was (2905.03±110.57)(cGy),which was higher than that of the non-bone marrow inhibition group (1938.21±91.02)(cGy), and the difference was statistically significant (P<0.05).The V20 value, V30 value and V40 value of the bone marrow suppression group were ( 63.56±4.59)%,(52.03±4.31)%,(25.16±2.92)%,and the V20 value, V30 value and V40 value of the non bone marrow suppression group were (42.17 ± 3.48)%,(23.98 ±3.02)%,(8.55 ± 1.90)%, and the differences between the two groups were statistically significant (P<0.05). Conclusions Radiation dose to sternum of esophageal cancer is closely related to bone marrow suppression, and the protection of sternum should be done well in esophageal cancer radiotherapy.
