rhEGF及清创术对口腔颌面部外伤患者血清EGF及炎症因子水平的影响

摘　　要：目的:研究美容清创缝合联合基因重组人表皮生长因子(recombinant human Epidermal Growth Factor,rhEGF)治疗口腔颌面部外伤患者的疗效及对患者血清表皮生长因子及炎症因子的影响。方法:回顾性分析60例口腔颌面部外伤患者的临床资料。根据其治疗方式分为观察组和对照组,每组30例。对照组:美容清创缝合术进行治疗;观察组:美容清创缝合术加rhEGF治疗。根据两组患者的检验资料对比其血清表皮生长因子及炎症因子水平,比较治疗12个月后的瘢痕治愈效果。结果:术前两组患者的TNF-α、IL-6、IL-1及表皮生长因子水平对比,差异无统计学意义(P>0.05),术后第2天、第5天,观察组患者的TNF-α、IL-6及IL-1值均显著低于对照组,患者的表皮生长因子水平显著高于对照组,两组比较差异均具有统计学意义(P<0.05);创口治愈结果显示,观察组显效18例(60.0%),对照组10例(33.3%)。观察组总有效28例(93.3%),大于对照组的22例(73.3%)。结论:在美容清创缝合基础上加rhEGF治疗口腔颌面部外伤患者具有更好的疗效,显著提高了患者的创口修复质量,大大减少了术后瘢痕的形成,值得临床推广使用。Objective To study the effect of cosmetic debridement and suture combined with recombinant human epidermal growth factor(recombinant human Epidermal Growth Factor,rhEGF) on oral and maxillofacial trauma patients and the effect on serum epidermal growth factor(EGF) and inflammatory factor(inflammatory factor) in patients with oral and maxillofacial trauma. Methods The clinical data of 60 patients with oral and maxillofacial trauma admitted in our hospital from August2013 to August 2016 were retrospectively analyzed. They were divided into observation group(n=30) and control(n=30).The control group was treated by cosmetic debridement and suture, and the observation group was treated with rhEGF on the basis of the control group. The serum levels of EGF and inflammatory factors were compared between the two groups and the healing effect of scar 12 months later. Results The results showed that there was no significant difference in the levels of IL-1 and EGF between the two groups before operation(P>0.05). The levels of TNF-α,IL-6 and IL-1 in the observation group were significantly lower than those in the control group on the 5 th day after operation, and the EGF levels in the observation group were significantly higher than those in the control group(P<0.05). The results showed that there were 18 cases(60.0%) in the observation group. There were 10 cases(33.3%) in the control group, and 28 cases(93.3%) in the observation group were more effective than 22 cases(73.3%) in the control group. Conclusion On the basis of cosmetic debridement and suture, rhEGF has a better therapeutic effect on oral and maxillofacial trauma. RhEGF can promote wound healing and is worthy of clinical application.
