口腔黏膜下纤维化和癌变患者口腔黏膜组织细胞凋亡指数及其对癌变发生发展的影响

摘　　要：目的分析口腔黏膜下纤维化和癌变患者口腔黏膜组织细胞凋亡指数及其对癌变发生发展的影响。方法选取宝鸡市人民医院48例口腔黏膜下纤维化患者,根据病理学分期分为早期组(14例)、中期组(18例)、晚期组(16例),另选取同期32例口腔黏膜下纤维化癌变患者为癌变组及30例健康体检者为对照组。取各组口腔内同一部位的黏膜组织,采用TUNEL法对其细胞凋亡指数进行检测,通过免疫组织化学法对口腔黏膜组织中B淋巴细胞瘤-2(Bcl-2)和Bcl-2相关X蛋白(Bax)的表达阳性率进行检测。结果晚期组和癌变组口腔黏膜组织中细胞凋亡指数较早期组、中期组及对照组均明显增高(P<0.05);癌变组细胞凋亡指数较晚期组明显增高(P<0.05)。癌变组口腔黏膜组织中Bcl-2蛋白表达阳性率较早期组、中期组及对照组均明显升高(P<0.05);晚期组Bcl-2蛋白表达阳性率较对照组明显升高(P<0.05)。癌变组口腔黏膜组织中Bax蛋白表达阳性率较早期组、中期组及对照组均明显升高(P<0.05);晚期组Bax蛋白表达阳性率较早期组明显升高(P<0.05)。结论随着口腔黏膜下纤维化程度的加重,口腔黏膜下纤维化与癌变患者口腔黏膜组织细胞凋亡指数明显增高,且随着病情程度的加重,口腔黏膜组织中Bcl-2和Bax蛋白表达阳性率明显升高,提示二者在口腔黏膜下纤维化和癌变的发生和发展的病理过程中可能起到重要作用。Objective To analyze the apoptotic index of oral mucosal tissue in patients with oral submucosal fibrosis and canceration and its influence on the occurrence and development of canceration.Methods 48 patients with oral submucosal fibrosis were divided into early group(14 cases),middle group(18 cases)and advanced group(16 cases)according to pathological stages.In the same period,32 patients with oral submucosal canceration were selected as canceration group and 30 healthy people as control group.The apoptotic index was detected by TUNEL,and the positive rates of B-cell lymphoma-2(Bcl-2)and Bcl-2 associated X protein(Bax)in oral mucosa were detected by immunohistochemistry.Results The apoptotic index of oral mucosa in advanced group and carcinogenesis group was significantly higher than that in early group,middle group and control group(P<0.05),and the apoptotic index in carcinogenesis group was significantly higher than that in advanced group(P<0.05).The positive rate of Bcl-2 protein expression in oral mucosa of cancer group was significantly higher than that of early group,middle group and control group(P<0.05),while that of late group was significantly higher than that of control group(P<0.05).The positive rate of Bax protein expression in oral mucosa of cancer group was significantly higher than that of early group,middle group and control group(P<0.05),while that of late group was significantly higher than that of early group(P<0.05).Conclusion With the aggravation of oral submucosal fibrosis,the apoptotic index of oral mucosa and oral mucosal tissues of patients with canceration increase significantly,and with the aggravation of the disease,the positive rates of Bcl-2 and Bax protein expression in oral mucosal tissues increase significantly,suggesting that they may play an important role in the occurrence and development of oral submucosal fibrosis and carcinogenesis.
