摘 要:目的 探讨人血清增殖诱导配体(APRIL)及胃蛋白酶原Ⅰ/胃蛋白酶原Ⅱ(PGR)在胃炎、胃癌中的诊断价值,检测APRILmRNA在胃部疾病中的表达情况。方法选取南京市第一医院接受胃镜检查的患者,选取胃镜结果为正常、非萎缩性胃炎、萎缩性胃炎、胃癌患者各80例。采用酶联免疫吸附试验(ELISA)检测四组患者的血清胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、APRIL、幽门螺杆菌(H.pylori)抗体的水平,计算PGR值,制作APRIL、PGR预测胃炎、胃癌的ROC曲线图,给出截断值,RT-PCR实验检测正常对照者、非萎缩性胃炎患者、萎缩性胃炎患者、胃癌患者的APRILmRNA表达水平的差异。结果ELISA实验显示,PGⅠ、PGR的数值从正常对照者、非萎缩性胃炎患者、萎缩性胃炎患者到胃癌患者逐渐降低,差异有统计学意义(P<0.05),APRIL、H.pylori抗体的检测值逐渐升高,差异有统计学意义(P<0.05)。RT-PCR实验显示,胃癌患者APRILmRNA的表达水平显著高于正常对照者、非萎缩性胃炎、萎缩性胃炎患者,差异有统计学意义(P<0.05)。结论APRIL、PGR是预测胃炎、胃癌的良好初筛指标,APRILmRNA的高表达可能与胃癌密切相关。Objective To explore the diagnostic value of a proliferation-inducing ligand(APRIL) and PGR in gastritis and gastric cancer, and to detect the expression of APRIL mRNA in gastric diseases. Methods Patients who underwent gastroscopy were chosen. Among them, there were 80 cases of normal controls, non-atrophic gastritis patients, atrophic gastritis patients and gastric cancer patients. The levels of serum pepsinogen Ⅰ(PGⅠ), pepsinogen Ⅱ(PGⅡ), APRIL, H.pylori antibody were detected by enzyme-linked immunosorbent assay(ELISA) in these four groups, and the PGⅠ/PGⅡ ratio(PGR) was calculated, the ROC curves about PGR and APRIL were made, then the cut off value was calculated, RT-PCR assay was used to detect the expression levels of APRIL mRNA in these four groups. Results ELISA showed that the values of PGⅠ and PGR were reduced gradually from normal controls to gastric cancer patients, the difference was statistically significant(P<0.05), the values of APRIL and H.pylori antibodies were increased gradually from normal controls to gastric cancer patients, the difference was aslo statistically significant(P<0.05). RT-PCR assay found the expression level of APRIL mRNA in gastric cancer patients was significantly higher than that in normal controls, non-atrophic gastritis and atrophic gastritis, and the difference was statistically significant(P<0.05). Conclusion APRIL and PGR are good screening indicators for gastritis and gastric cancer. The expression level of APRIL mRNA may be closely related to the progress of gastric cancer.
