胸腔镜手术与开胸手术治疗胸腺瘤合并重症肌无力的效果比较

摘　　要：目的对比分析电视胸腔镜手术与胸骨正中劈开手术在治疗胸腺瘤合并重症肌无力的临床效果。方法选取开胸及胸腔镜治疗胸腺瘤及合并重症肌无力患者各30例,比较两组患者的临床治疗效果。结果电视胸腔镜组在手术切口长度、手术时间、术后疼痛、术中出血量、术后住院天数与胸骨正中劈开组比较,差异具有统计学意义(P<0.01);开胸组有1例并发重症肌无力危象。电视胸腔镜组的总有效率与胸骨正中劈开组比较,差异无统计学意义(P>0.05)。结论胸腔镜较开胸手术具有创伤小、恢复快等显著优势,胸腺扩大切除术值得临床推广应用。Objective To compare the clinic effects of VATS and thymectomy in the treatment of thymoma and myasthenia gravis.Methods From February 2006 to July 2014 the patients from author's hospital were divided into two groups,30 cases in each group.The clinical data confirmed to be thymoma and myasthenia gravis preoperatively was retrospectively analyzed.Results VATS group was superior to those of thoracotomy group(P<0.01)in the incision length,the operative time,postoperative pain,bleeding volume during operation,postoperative hospitalization days.In VATS group,One case complicated with MG crisis was found in sternum split group.The remission rate there was no significant difference between two group(P>0.05).Conclusion Video-assisted thoracic surgery has the superiority of less trauma and quicker recovery compared with transsternal procedures.Extensive thymectomy with VATS is safe and feasible in the surgical treatment of MG.
