摘 要:目的探讨血清总胆汁酸(TBA)、前白蛋白(PA)及甲状腺激素联合检测在慢性肝病中的临床价值。方法选取2016年8月~2018年8月我院收治慢性肝病患者60例,根据Child-Pugh肝功能分级标准分为A1组28例和A2组32例。另于同期选取40例健康体检者作为对照组。测定体检者血清TBA、PA及甲状腺激素水平。结果三组之间血清TBA、TSH水平比较,差异有统计学意义(P <0.05),其中均以A2组最高、A1组其次、对照组最低;三组之间血清PA、TT3、TT4、FT3、FT4水平比较,差异有统计学意义(P <0.05),其中以对照组最高、A2组其次、A1组最低。三组之间血清TBA、PA、TT3、TT4、TSH、FT3、FT4水平异常发生率比较,差异有统计学意义(P <0.05),其中均以A2组最高、A1组其次、对照组最低。结论血清TBA、PA及甲状腺激素联合检测可反映慢性肝病患者肝细胞和肝功能受损程度,对慢性肝病的预后的判断具有一定意义。Objective To explore clinical values of combined detection of serum total bile acid(TBA), prealbumin(PA) and thyroid hormones in chronic liver disease. Methods 60 patients with chronic liver disease who were admitted and treated in our hospital from August 2016 to August 2018 were selected. According to Child-Pugh liver function grading standard, they were divided into A1 group with 28 cases and A2 group with 32 cases. In addition, 40 healthy people who underwent physical examinations in the same period were selected as the control group. Serum TBA, PA and thyroid hormone levels of the objects were measured. Results There were statistically significant differences in serum TBA and TSH among three groups(P < 0.05). Serum TBA and TSH levels were highest in the A2 group, the second in the A1 group and the lowest in the control group. There were statistically significant differences in PA, TT3, TT4, FT3 and FT4 among three groups(P < 0.05). PA, TT3, TT4, FT3 and FT4 levels were highest in the control group, the second in the A2 group and the lowest in the A1 group. There were statistically significant differences in incidence of abnormality of TBA, PA, TT3, TT4, TSH, FT3 and FT4 levels among three groups(P<0.05).The incidence of abnormality of TBA, PA, TT3, TT4, TSH, FT3 and FT4 levels were highest in the A2 group, the second in the A1 group and the lowest in the control group. Conclusion The combined detection of serum total bile acid, prealbumin and thyroid hormones can reflect the impairment degree of hepatocyte and liver function in patients with chronic liver disease, which has certain significance in judging the prognosis of t chronic liver disease.
