摘 要:目的:研究原发性浸润性乳腺癌(非特指)中GATA结合蛋白3(GATA-3)、叉头框架蛋白A1(FOXA1)及雌激素受体(ER)的表达、相互关系及其预后意义.方法:收集乳腺癌改良根治标本138例,依据ER、孕激素受体(PR)、Ki-67及HER-2表达,分为Luminal A型、Luminal B型、Her-2过表达型及三阴型乳腺癌.采用免疫组化EnVision法检测138例非特殊型原发性浸润性乳腺癌组织中GATA-3、FOXA1和ER的蛋白表达.结果:GATA-3的总体阳性表达率为66.7%(92/138),其中Luminal A型阳性率90.0%(45/50)、Luminal B型阳性率74.1%(43/58)、Her-2过表达型阳性率30.0%(3/10)、三阴型阳性率5.0%(1/20);FOXA1的总体阳性表达率为40.6%(56/138),其中Luminal A型阳性率78.0%(39/50)、Luminal B型阳性率27.6%(16/58)、Her-2过表达型阳性率10.0%(1/10)、三阴型阳性率0(0/20).GATA-3与FOXA1在Luminal A型组的表达率显著高于其他亚型(P<0.01).GATA-3与FOXA1的表达均与肿瘤大小、Nottingham预后指数、组织学分级呈负相关(P<0.01).GATA-3和FOXA1的阳性表达率与基底样表型标记(CK5/6)及表皮生长因子受体(EGFR)的表达率均具有很强的负相关(P<0.01).GATA-3与FOXA1在ER/PR阳性乳腺癌中的阳性率明显高于ER/PR阴性乳腺癌中的阳性率(P<0.01),而且GATA-3和FOXA1的表达均与ER、PR直接相关(P<0.01).结论:GATA-3与FOXA1在乳腺癌中与ER表达密切相关,可以作为乳腺癌预后良好的指标;另外对于GATA-3表达的乳腺癌,FOXA1可用于危险度分层.Objective:To investigate the expressions of GATA-3,forkhead-box protein A1(FOXA1)and ER in primary invasive breast cancer(not otherwise specified),the correlations among these three proteins,and their significant for prognosis.Methods:The tumor samples were collected form 138 patients with invasive breast cancer and were divided into Luminal A,Luminal B,Her-2 overexpression,and triple negative subtype group according to the expression of estrogen receptor,progesterone receptors,Ki-67,and Her-2.EnVision method was used to detect the expression of GATA-3,FOXA1 and ER in the samples.Results:GATA-3 overall positive expression rate was 66.7%(92/138),with 90.0%(45/50)in Luminal A subtype,74.1%(43/58)in Luminal B subtype,30.0%(3/10)in Her-2 overexpression subtype,and 5.0%(1/20)in triple negative subtype.FOXA1 was detected in 40.6%(56/138)of these cases,with 78.0%(39/50)in Luminal A subtype,27.6%(16/58)in Luminal B subtype,10%(1/10)in Her-2 overexpression subtype,0%(0/20)in triple negative subtype(P<0.01).The differences among these four groups were statistically significance(P<0.01).GATA-3 and FOXA1 positive rates in the luminal A subtype were the highest among four groups(P<0.01).The expressions of both GATA-3 and FOXA1 are negatively correlated with tumor size,Nottingham Prognostic Index,histological grade(P<0.01).FOXA1 positive rate and GATA-3 positive rate are negatively correlated with the expression of basal-like phenotype markers CK5/6 and EGFR(P<0.01).GATA-3 positive rate and FOXA1 positive rate are significantly higher in ER/PR+than that in ER/PR-breast cancer(P<0.01).Both GATA-3 and FOXA1 were directly associated with ER and PR(P<0.01).Conclusion:FOXA1 and GATA-3 closely associate with ER expression in breast cancer,which can serve as prognostic indicators for invasive breast cancer.In addition,the expression of FOXA1 can be used for risk stratification of GATA-3 positive patients.
