微小切口进路坚固内固定治疗下颌骨骨折的疗效及安全性

摘　　要：目的探讨微小切口进路坚固内固定治疗下颌骨骨折的疗效及安全性。方法选取我院2013年9月至2016年2月我院收治的330例下颌骨骨折患者,按手术方法的不同分为观察组和对照组,各165例,观察组采取微小切口进路坚固内固定治疗,对照组采取切开复位内固定治疗,比较2组患者临床治疗效果。结果观察组患者总有效率明显高于对照组患者(χ^2=4.5744,P<0.05);观察组患者Ⅰ期愈合率明显高于对照组患者(χ^2=12.5656,P<0.05);观察组患者神经麻痹、咬合不佳和感染发生率均低于对照组患者(χ^2=4.6082,8.8072,5.5971,P<0.05);观察组患者软组织肿胀消退时间、骨痂形成时间、开口度恢复时间均短于对照组患者(t=2.2111、1.7894、1.9851,P<0.05)。结论相较于切开复位内固定术,微小切口进路坚固内固定术治疗下颌骨骨折能够取得更为满意的疗效,且具有并发症少、恢复效果佳等优点。Objective To investigate the efficacy and safety of microincision approach for internal fixation of mandibular fracture.Methods A total of 330 patients with mandibular fracture in our hospital from September 2013 to February 2016 were selected and randomly divided into the study group and the control group,with 165 cases in each group.The study group was treated with microincision approach for solid internal fixation,and the control group was treated with open reduction and internal fixation.The clinical treatment effect of the two groups was compared.Results The total effective rate of the study group was significantly higher than that of the control group(χ^2=4.574 4,P<0.05).The healing rate of stageⅠin the study group was significantly higher than that in the control group(χ^2=12.565 6,P<0.05).The incidence of nerve palsy,malocclusion and infection in the study group were lower than those in the control group(χ^2=4.608 2,8.807 2,5.597 1,P<0.05).The time of soft tissue swelling regression,callus formation and opening recovery were shorter in the study group than those in the control group(t=2.211 1,1.789 4,1.985 1,P<0.05).Conclusion Compared with open reduction and internal fixation,the treatment of mandibular fracture by microincision approach with solid internal fixation can achieve more satisfactory efficacy and has the advantages of fewer complications and better recovery effect.
