液基细胞学联合HPV分型在宫颈癌早期筛查中的意义

摘　　要：目的探究液基细胞学(TCT)联合人乳头瘤病毒(HPV)分型在宫颈癌早期筛查中的意义。方法选取2014年5月~2018年5月在我院进行宫颈癌筛查的患者4000例,观察并比较不同检测方法的检出结果。结果HPV灵敏度为81.00%(1215/1500),特异度为95.48%(2387/2500),阳性预测值为91.49%(1215/1328),阴性预测值为89.33%(2387/2672);TCT灵敏度为65.73%(986/1500),特异度为86.36%(2159/2500),阳性预测值为74.30%(986/1327),阴性预测值为80.77%(2159/2673);2者联合灵敏度为86.53%(1298/1500),特异度为95.72%(2393/2500),阳性预测值为92.38%(1298/1405),阴性预测值为92.22%(2393/2595),TCT联合HPV-DNA分型检测的灵敏度、特异度、阳性预测值、阴性预测值均显著高于单独应用TCT,灵敏度、阴性预测值显著高于单独HPV-DNA分型检测(P<0.01)。结论TCT联合HPV-DNA分型检测可以提高诊断宫颈癌检出率,对宫颈癌的早发现、早治疗有重要临床意义。Objective To explore the significance of liquid-based cytology(TCT)combined with human papillomavirus(HPV)typing in early screening of cervical cancer.Methods 4000 patients who underwent cervical cancer screening in our hospital from May 2014 to May 2018 were selected to observe and compare the results of different detection methods.Results The positive detection rate of HPV in cervical cancer increased with the change of cervical cancer.The positive predictive value of HPV in cervical cancer was 91.49%(1215/1328),the positive predictive value of TCT was 91.49%(1215/1328),and the combined positive predictive value of HPV and TCT was 92.38%.The sensitivity,specificity,positive predictive value and negative predictive value of TCT combined with HPV-DNA typing were significantly higher than those of TCT alone,and the negative predictive value was significantly higher than that of HPV-DNA typing alone(P<0.01).Conclusion TCT combined with HPV-DNA typing can improve the detection rate of cervical cancer,and has important clinical significance for early detection and treatment of cervical cancer.
