摘　　要：目的:探讨吡拉西坦联合氨磺必利对以阴性症状为主的精神分裂症患者前瞻性记忆及认知功能的影响。方法:80例以阴性症状为主的精神分裂症患者随机分为观察组及对照组;两组在采用认知矫正联合氨磺必利(0.4~0.8g/d)治疗基础上,观察组加用吡拉西坦治疗(2.4g/d),疗程3个月;治疗前后给予患者阳性与阴性症状量表(PANSS)评估病情;采用双任务研究范式对患者进行基于时间及基于事件前瞻记忆测试;采用连线实验A(TMT-A)、数字广度测验-顺背(DS)、语次流畅性测验(VF)、威斯康辛卡片分类测验(WCST)对认知功能进行评估。结果:治疗后两组PANSS评分均明显降低,且观察组显著低于对照组(P均<0.01);治疗后两组基于时间及基于事件的前瞻记忆评分显著升高,且观察组明显高于对照组(P均<0.05);VF、DS、TMT-A及WCST评分明显改善,且两组差异具有统计学意义(P均<0.05)。结论:吡拉西坦联合氨磺必利可明显改善以阴性症状为主的精神分裂症患者的病情、前瞻记忆能力及认知功能。Objective:To investigate the influence of piracetam combined with amisulpride on prospective memory and cognitive function in schizophrenic patients with negative symptoms. Method:Eighty schizophrenic patients with negative symptoms were randomly divided into observation group and control group.One the basic of cognitive correction combined with amisulpride(0.4-0.8 g/d) in the two groups,the observation group took piracetam(2.4 g/d) for 3 months.Before and after treatment,the condition,prospective memory and cognitive function of the patients were assessed by Positive and Negative Symptom Scale(PANSS),time-and event-based prospective memory,Trail Making Test(TMT-A),Digital Span Test-straight back(DS),Verbal Fluency Test(VF),Wisconsin Card Classification Test(WCST). Results:After treatment,the scores of PANSS in the two groups significantly decreased,and these scores in the observation group were lower than those in the control group(all P<0.01);the test scores of time-and event-based prospective memory of the two groups significantly increased,and these scores in the observation group were higher than those in the control group(all P<0.05);VF,DS,TMT-A,WCST scores improved, and there were significant differences between two groups(all P<0.05). Conclusion: Piracetam combined with amisulpride can significantly improve the conditions,prospective memory and cognitive function in schizophrenic patients with negative symptoms.
