109例癫痫患者丙戊酸钠血药浓度分析

摘　　要：目的探讨不同丙戊酸钠(VAP)血药浓度对癫痫患者肝、肾功能的影响。方法收集109例服用VAP癫痫患者服药后的血清样本,按照血清VAP稳态药物浓度分为<50、50~100和>100μg·mL^-1三组,分别检测并比较各组肝、肾功能指标。结果109例癫痫患者服药后,VAP<50μg·mL^-1者30例(占27.5%),VAP50~100μg·mL^-1者40例(占36.7%),VAP>100μg·mL^-1者39例(占35.8%)。不同VAP浓度的癫痫患者的血清谷丙转氨酶(ALT)、天门冬氨酸氨基转氨酶(AST)、r-谷氨酰基转移酶(GGT)、碱性磷酸酶(ALP)异常率差异均有统计学意义(P均<0.05),三组癫痫患者肾功能均正常。结论VAP血药浓度存在个体差异,需定期对患者进行血清VAP浓度和肝功能监测。Objective To investigate the effect of serum valproate(VAP) concentration on liver and kidney function of epileptic patients. Methods Serum samples of 109 epilepsy patients after taking VAP were collected and divided into < 50, 50~100 and > 100μg·mL^-1 groups according to the steady-state concentration of VAP. The liver and kidney function indexes of each group were detected and compared. Results Among the 109 samples, 30 of them got VAP lower than 50μg·mL^-1,40 of them got VAP between 50μg·mL^-1 and 100μg·mL^-1,and 39 of them got VAP higher than 100μg·mL^-1. The results showed that the levels of ALT,AST,GGT and ALP in epileptic patients with different concentration of VAP were significantly different(P<0.05),and the renal function was normal in the three groups. Conclusion The blood drug concentration of VAP has individual difference,so serum VAP concentration and liver function should be monitored regularly in order to guide clinical and individualized use of drugs for patients.
