摘 要:目的探讨miRNA-200b、miRNA-200c和miRNA-141在肺癌患者血清中的表达情况及其临床意义。方法选取96例肺癌患者(肺癌组)和95例同期健康体检者(健康对照组)。采用实时荧光定量聚合酶链反应(qRT-PCR)检测两组受试者血清miRNA-200b、miRNA-200c和miRNA-141的相对表达量,采用受试者工作特征(ROC)曲线分析血清miRNA-200b、miRNA-200c和miRNA-141对肺癌的诊断效能,及其与肺癌患者的临床特征及预后的关系。结果肺癌组患者血清miRNA-200b、miRNA-200c和miRNA-141的相对表达量均明显低于健康对照组(P﹤0.01);miRNA-200b、miRNA-200c和miRNA-141诊断肺癌的ROC曲线下面积分别为0.749、0.746和0.827,灵敏度分别为78.15%、54.71%、84.13%,特异度分别为68.41%、84.65%、75.32%;不同TNM分期的肺癌患者血清miRNA-200b、miRNA-200c的表达情况比较,不同病理类型、鳞状细胞癌抗原(SCC)水平的肺癌患者血清miRNA-141的表达情况比较,差异均有统计学意义(P﹤0.05);miRNA-200b、miRNA-200c、miRNA-141高表达患者的3年生存率分别为62.16%、58.14%、63.64%,均分别高于miRNA-200b、miRNA-200c、miRNA-141低表达患者的23.73%、22.64%、25.40%(P﹤0.05)。结论miRNA-200b、miRNA-200c和miRNA-141在肺癌患者血清中的相对表达量较低,对肺癌诊断及预后评估具有一定的价值。Objective To detect the expressions of miRNA-200b, miRNA-200c and miRNA-141 in serum of patients with lung cancer, and to explore their clinical significance. Method The research included 96 cases of lung cancer and 95 healthy volunteers during the same period as lung cancer group and healthy control group, respectively. Quantitative real time-polymerase chain reaction (qRT-PCR) was used to detect the expression levels of miRNA-200b, miRNA-200c and miRNA-141 in the serum of all subjects, receiver operating characteristic (ROC) curve was plotted to investigate the diagnostic efficiency of these markers for lung cancer, and to discuss the relationship with the clinical characteristic and prognosis of lung cancer patients. Result The levels of serum miRNA-200b, miRNA-200c and miRNA-141 in lung cancer group were significantly lower than those in healthy control group, the differences were statistically significant (P< 0.01). The area under the ROC curve of miRNA-200b, miRNA-200c and miRNA-141 were 0.749, 0.746 and 0.827, their sensitivity were 78.15%, 54.71%, and 84.13%, and specificity were 68.41%, 84.65%, and 75.32%, respectively;The serum miRNA-200b and miRNA-200c expression among lung cancer patients with different TNM stages, and the serum miRNA-141 expression in lung cancer patients with different pathological types and squamous cell carcinoma antigen (SCC) lever were of statistically significant differences (P<0.05). Survival analysis result showed that the 3-year survival rates of patients with high expression level of miRNA-200b, miRNA-200c, and miRNA-141 were 62.16%, 58.14%, and 63.64%, which were significantly higher than the 23.73%, 22.64%, and 25.40% in patients with low expression of the three markers (P<0.05). Conclusion miRNA-200b, miRNA-200c and miRNA-141 are of relative low expression level in the serum of lung cancer patients, indicating their value in evaluating the diagnosis and prognosis of lung cancer.
