摘　　要：目的探讨抗精神病药物帕利哌酮治疗首发精神分裂症患者血清中白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α、钙结合蛋白(S100β)水平的变化,为精神分裂症的免疫发病机制及治疗提供新的思路。方法用酶联免疫吸附试验(ELISA)法测定124例首发精神分裂症患者、78名健康对照者血清3种蛋白因子的浓度;并根据阳性与阴性症状量表(PANSS)将患者分为临床疗效的有效组和无效组;通过Pearson相关分析判定临床疗效与血清3种蛋白因子的相关性。结果首发精神分裂症患者组血清IL-6、TNF-α和S100β浓度均高于健康对照组,差异有统计学意义(P<0.05);与治疗前相比,治疗后血清S100β水平明显降低,差异具有统计学意义(P<0.05),而血清IL-6和TNF-α水平差异无统计学意义(P>0.05);Pearson相关分析得出,血清S100β表达水平和临床疗效间呈负相关(r=-0.435,P<0.05)。结论IL-6、TNF-α和S100β与精神分裂症的的发生密切相关;血清S100β表达水平可能为精神分裂症患者帕利哌酮临床疗效的预测指标。Objective By investigating the changes of serum interleukin(IL)-6,tumor necrosis factor(TNF)-αand S100βlevels before and after paliperidone treatment in patients with first-episode schizophrenia to provide new ideas for the immune pathogenesis and treatment of schizophrenia.Methods The levels of serum IL-6,TNF-αand S100βwere measured by the enzyme linked immunosorbent assay(ELISA)in 124 inpatients with schizophrenia and 78 healthy controls.The efficacy of paliperidone medication was evaluated by PANSS scale.Results The levels of serum in the schizophrenia case group were significantly higher than those in control group(P<0.05).After treatment,the level of serum S100βwas significantly declined(P<0.05).The change of IL-6 and TNF-αin serun were no significant different(P>0.05).According to Pearson correlation analysis,there was a negative correlation between serum concentration of S100βand clinical efficacy to paliperidone(r=-0.435,P<0.05).Conclusion IL-6,TNF-αand S100βare closely related to the occurrence of first-episode schizophrenia.The levels of serum S100βcan be used as a biological marker of the clinical efficacy to paliperidone in schizophrenia.
