摘  要:目的探讨肺癌患者CT扫描影像学特征与特异性标记物的相关性。方法选取2017年1月至2018年3月本院收治的60例原发性肺癌患者为研究组,同期健康志愿者60名为对照组。使用美国GE 64排螺旋CT进行增强扫描,收集影像学特征指标(肿瘤大小、病灶部位、坏死和空洞征象,毛刺征象,深分叶征象);使用C7多肿瘤标记物对应的检测试剂盒对7种特异性肿瘤标记物进行定量检测。比较影像学特征和特异性标记物的相关性。结果研究组CA199为(86.83±62.16)kU/L、CA125为(106.35±94.15)kU/L、CA242为(28.01±11.37)kU/L、癌胚抗原为(1.62±0.86)μg/L、神经元特异性烯醇化酶(NSE)为(2.30±1.46)μg/L、人绒毛膜促性腺激素(HCG)为(0.19±0.11)μg/L、血清铁蛋白为(263.52±105.61)μg/L,明显高于对照组,差异均有统计学意义(均P<0.05)。肿瘤最大径>5 cm、有坏死和空洞、有毛刺征象、有深分叶征象的肺癌患者血清CA199、CA125、CA242、癌胚抗原、NSE、HCG、血清铁蛋白含量明显高于肿瘤最大径≤5 cm、无坏死和空洞、无毛刺征象、无深分叶征象的肺癌患者,差异均有统计学意义(均P<0.05)。Pearson相关性分析显示,肿瘤大小与CA199、CA125、CA242、癌胚抗原、NSE、HCG、血清铁蛋白呈明显正相关(均P<0.05)。结论肺癌CT扫描影像学特征中的肿瘤大小、坏死和空洞征象、毛刺征象、深分叶征象与C7多肿瘤标记物的表达具有相关性;二者联合应用有利于提高肺癌诊断的准确性。Objective To investigate the correlation between CT imaging features and specific markers in patients with lung cancer.Methods From January 2017 to March 2018, 60 patients with primary lung cancer were selected as the study group and 60 healthy volunteers as the control group. GE 64-slice spiral CT was used for enhanced scanning to collect imaging features(tumor size, lesion location, necrosis and cavity signs, burr signs, deep lobulation signs); and C7 multi-tumor markers corresponding detection kits were used for quantitative detection of seven specific tumor markers. Results The CA199, CA125, CA242, CA199, CEA, NSE, HCG and serum ferritin in the study group were(86.83 +62.16) kU/L,(106.35 +94.15) kU/L,(28.01 +11.37) kU/L,(1.62 +0.86)μg/L,(2.30 +1.46) μg/L,(0.19 +0.11) μg/L and(263.52 +105.61) μg/L, respectively, which were significantly higher than those in the control group. Academic significance(P<0.05). The serum levels of CA199, CA125, CA242, CEA, NSE, HCG and serum ferritin in lung cancer patients with tumor maximum diameter>5 cm, necrosis and cavity, burr and deep lobulation were significantly higher than those in lung cancer patients with tumor maximum diameter≤5 cm, no burr and no deep lobulation. The differences were statistically significant. There was no significant difference in the content of specific markers between peripheral lung cancer and central lung cancer(P>0.05). Pearson correlation analysis showed that tumor size was positively correlated with CA199, CA125, CA242, CEA, NSE, HCG and serum ferritin(P<0.05).Conclusion The size, necrosis and cavity, burr and deep lobulation of lung cancer are significantly correlated with the expression of C7 multi-tumor markers, and the combination of them is helpful to improve the diagnostic accuracy of lung cancer.
