高强度超声聚焦治疗黏膜下子宫肌瘤的临床效果

摘　　要：目的探讨高强度超声聚焦(HIFU)治疗黏膜下子宫肌瘤的临床效果.方法选取2015年1月~2016年1月在佛山市禅城区中心医院治疗的85例黏膜下子宫肌瘤患者的临床资料,随机分为HIFU组(30例)、宫腔镜组(30例)、对照组(25例).HIFU组采用HIFU治疗,宫腔镜组采用宫腔镜电切治疗,对照组采用腹腔镜微创手术治疗.比较三组的住院时间、月经出血量、持续时间、月经复潮时间;随访2年观察三组的妊娠情况.结果HIFU组住院时间、月经出血持续时间短于宫腔镜组和对照组(P<0.05);HIFU组疼痛评分低于宫腔镜组和对照组,差异有统计学意义(P<0.05).HIFU组月经异常发生率低于宫腔镜组、对照组,异有统计学意义(P<0.05).HIFU组妊娠并发症低于宫腔镜组、对照组,差异有统计学意义(P<0.05).三组的月经复潮时间比较,差异无统计学意义(P>0.05).结论HIFU治疗黏膜下子宫肌瘤对生育功能影响较小,接受程度较高,值得临床推荐.Objective To explore the clinical effect of high intensity focused ultrasound (HIFU) in the treatment of submucosal hysteromyoma. Methods A total of 85 cases of submucous uterine leiomyoma treated in Chancheng District Central Hospital of Foshan City from January 2015 to January 2016 were selected. They were randomly divided into HIFU group (30 cases), hysteroscopy group (30 cases) and control group (25 cases). HIFU group was treated by HIFU, hysteroscopy group was treated by hysteroscopy electrotomy, and control group was treated by laparoscopic minimally invasive surgery. The hospitalization time, menstrual bleeding volume, duration and menstrual rejuvenation time of three groups were compared, and lhe pregnancy status of each group was observed after 2 years of follow-up. Results The hospitalization time of HIFU group was shorter than that of hysteroscopy group and control group. The duration of menstrual bleeding in HIFU group was shorter than that in hysteroscopy group and control group. The pain score of HIFU group was lower than that of hysteroscopy group and control group, the difference was statistically significant (P<0.05). The incidence of abnormal menstruation in HIFU group was lower than that in hysteroscopy group and control group (P<0.05). The pregnancy complications in HIFU group were significantly lower than those in hysteroscopy group and control group (P<0.05). There was no significant difference in the time of menstruation between the three groups (P>0.05). Conclusion The HIFU treatment of submucosal uterine leiomyoma has less impact on reproductive function and higher acceptance, and it is worthy of clinical recommendation.
