摘 要:目的探讨高角型Ⅱ类1分类下颌后缩患者舌体与下颌骨形态及位置之间的相关关系。方法选取高角型安氏Ⅱ类1分类下颌后缩成年患者45例,通过CBCT三维重建测量其舌体形态(舌长度,舌高度),舌体位置(舌水平位置T-S’,舌垂直位置S-S’)及下颌骨形态(下颌升支长度Co-Go,下颌骨上端的宽度Co-Co’,下颌体部的长度Go-Me,下颌骨下端的宽度Go-Go’,下颌体部的水平开张度∠Go-Me-Go’,下颌垂直的打开量∠Co-Me-Co’,下颌角的大小∠Co-Go-Me)。采用Pearson相关分析法分析舌体形态及位置与下颌骨形态的相关性。结果下颌升支长度Co-Go与舌长度呈正相关(r=0.653,P<0.05),与舌高度呈负相关(r=-0.627,P<0.05),与舌水平位置T-S’呈正相关(r=0.550,P<0.05)。下颌角∠Co-Go-Me与舌长度呈负相关(r=-0.557,P<0.05),与舌高度呈正相关(r=0.519,P<0.05),与舌水平位置T-S’呈负相关(r=-0.495,P<0.05),其余指标之间未见相关性。结论高角型Ⅱ类1分类下颌后缩患者舌体形态位置与下颌骨形态有相关性,随着舌体位置后移及舌高度增加,下颌升支高度减小,下颌角增大。Objective To investigate the correlation between tongue and mandible in high-angle classⅡdivision 1 malocclusion with mandibular retrusion.Methods A total of 45 patients with high-angle classⅡdivision 1 malocclusion were selected.The shape of tongue(length of tongue,height of tongue)and position of tongue(horizontal position of tongue T-S’,vertical position of tongue S-S’)and mandibular shape(mandibular ramus length Co-Go,mandibular upper width Co-Co’,mandibular length Go-Me,mandibular lower width Go-Go’,mandibular horizontal opening Go-Me-Go’,mandibular vertical opening Co-Me-Co’,the size of mandibular angle Co-Go-Me)were measured by CBCT 3D reconstruction.Pearson correlation analysis was used to analyze the correlation between the shape and position of tongue and the shape of mandible.Results The length of mandibular ramus was positively correlated with the length tongue(r=0.653,P<0.05)and horizontal position of tongue T-S’(r=0.550,P<0.05),but negatively correlated with height of tongue(r=-0.627,P<0.05).Mandibular angle∠Co-Go-Me was negatively correlated with length of tongue(r=-0.557,P<0.05)and horizontal position of tongue T-S’(r=-0.495,P<0.05),but positively correlated with height of tongue(r=0.519,P<0.05).There was no correlation between the other indicators.Conclusion The shape and position of the tongue are correlated with the shape of mandible in patients with high-angle classⅡdivision 1 malocclusion with mandibular retrusion.As the tongue position moves backward and tongue height increases,the mandibular ramus is reduced and mandibular angle is enlarged.
