正畸微型种植体对安氏Ⅱ类错颌畸形患者牙周炎症及MMP-2、MMP-9的影响

摘　　要：目的探讨正畸微型种植体对安氏Ⅱ类错颌畸形患者牙周炎症及金属基质蛋白酶2(MMP-2)、金属基质蛋白酶9(MMP-9)的影响,为临床治疗提供参考依据。方法选择2016年9月至2018年12月西安市中心医院口腔科接诊的安氏Ⅱ类错颌畸形患者108例。按随机数表法将所有患者分为对照组和观察组,每组54例,对照组患者予以传统正畸治疗,观察组患者予以正畸微型种植体治疗。治疗3个月后,观察并比较两组患者治疗前后的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)等炎症因子水平、治疗后并发症发生率以及MMP-2和MMP-9的表达水平。结果治疗前两组患者的IL-1β、IL-6、IL-8水平比较差异均无统计学意义(P>0.05);治疗3个月后两组患者的IL-1β、IL-6、IL-8水平均有明显提高,但观察组患者上述炎症因子提升的幅度明显低于对照组,差异均有统计学意义(P<0.05);观察组患者治疗后并发症发生率为3.70%,明显低于对照组的9.26%,差异具有统计学意义(P<0.05);治疗前两组患者的MMP-2、MMP-9表达水平比较差异均无统计学意义(P>0.05);治疗3个月后两组患者MMP-2、MMP-9表达均明显提高,观察组患者的MMP-2和MMP-9表达水平分别为(2.01±0.19)灰度值、(1.61±0.20)灰度值,其提升幅度明显低于对照组的(2.55±0.15)灰度值、(3.12±0.32)灰度值,差异均有统计学意义(P<0.05)。结论正畸微型种植体矫正治疗安氏Ⅱ类错颌畸形患者取得满意疗效,其不仅降低炎症因子水平,使患者牙周炎性微环境得到有效缓解,同时使治疗后并发症发生率有效降低,且创造极佳条件促使患者牙周组织修复,值得临床借鉴并推广实施。Objective To investigate the effects of orthodontic micro-implants on periodontal inflammation,metalloproteinase-2(MMP-2) and metalloproteinase-9(MMP-9) in patients with Class Ⅱ malocclusion, and provide reference for clinical treatment. Methods A total of 108 patients with Class Ⅱ malocclusion, who admitted to Department of Stomatology, Xi’an Central Hospital from September 2016 to December 2018 were selected and divided into the control group and study group according to random number table method, with 54 cases in each group. The patients in the control group were treated with traditional orthodontic treatment, and the patients in the study group were treated with orthodontic micro-implants. After 3 months of treatment, the levels of inflammatory factors such as interleukin-1β(IL-1β), interleukin-6(IL-6), and interleukin-8(IL-8), the incidence of post-treatment complications and the expression of MMP-2 and MMP-9 were observed and compared between the two groups before and after treatment. Results There were no significant differences in IL-1 β, IL-6 and IL-8 levels between the two groups before treatment(P>0.05);after 3 months of treatment, the levels of IL-1β, IL-6 and IL-8 in the two groups were significantly increased, but the increase of the above inflammatory factors in the study group was significantly lower than that in the control group(P<0.05);the incidence of postoperative complication in the study group was 3.70%, which was significantly lower than 9.26% in the control group(P<0.05);there was no significant difference in the expression levels of MMP-2 and MMP-9 between the two groups before treatment(P>0.05);after 3 months of treatment, the expressions of MMP-2 and MMP-9 were significantly increased in the two groups, and the expression levels of MMP-2 and MMP-9 in the study group were 2.01 ± 0.19 and 1.61 ± 0.20, respectively, which were significantly lower than corresponding 2.55 ± 0.15 and 3.12 ± 0.32 of the control group(P<0.05).
