摘  要:甲状腺外科手术中,喉返神经和喉上神经功能保护是手术成败的关键,也是专科医生培训及低年资医生甲状腺手术临床教学的重点与难点。应用术中神经监测技术(intraoperative neuromonitoring,IONM),术中导航快速识别喉返神经和喉上神经走行,并且实时检测神经功能及判断神经损伤机制,在外科教学中发挥积极的作用。应用术中神经检测可辅助初学者顺利寻找和定位喉返神经,提升术者操作信心,提高喉返神经识别率,降低手术操作盲目性。同时术中实时的神经检测会强调和加深对喉上神经保护的意识,并积累临床经验,掌握神经功能保护的手术技巧,提高手术技能,缩短学习曲线,减少医疗差错及纠纷。In thyroid surgery, the functional protection of recurrent laryngeal nerve and superior laryngeal nerve is the key step of surgery. It is also the focus and difficulty of specialist training and clinical teaching of thyroid surgery for junior doctors. Intraoperative neuromonitoring (IONM) is used to quickly identify the location of recurrent laryngeal nerve and superior laryngeal nerve, and to detect nerve function and determine the mechanism of nerve injury in real time. IONM plays an active role in surgical teaching. Intraoperative nerve detection can help beginners find and locate the recurrent laryngeal nerve easily, enhance the confidence of the operator, improve the recognition rate of recurrent laryngeal nerve, and reduce the blindness of the operation. At the same time, intraoperative realtime nerve detection will emphasize and deepen the awareness of superior laryngeal nerve protection, accumulate clinical experience, master surgical skills of nerve function protection, improve surgical skills, shorten learning curve, and reduce medical errors and disputes.
