摘　　要：目的探讨原发性膀胱小细胞癌的诊治手段及疗效,提高对膀胱小细胞癌的认识。方法回顾性分析12例膀胱小细胞癌的临床资料及术后随访,并复习相关文献。结果 12例患者,男9例,女3例,平均年龄(64.00±1.24)岁。分期T2N0M0 4例,T3N0M0 4例,T4N0M0 3例,T4N1M1 1例。术前2疗程化疗+根治性膀胱切除2例,术前2疗程化疗+姑息性膀胱切除1例,肿瘤电切+术后膀胱灌注4例,单纯肿瘤电切2例,膀胱部分切除2例,电切+化疗1例。9例死于肿瘤复发转移,平均生存时间(10.7±1.92)个月;1例术后6个月死于心脏疾病;2例至今已经分别存活3、7年。结论膀胱小细胞癌恶性程度高,预后差,治疗较为困难,手术+联合化疗是主要治疗方法。Objective To study the diagnosis, treatment and prognosis of primary small cell carcinoma of the urinary bladder, so as to improve the understanding of it. Methods A retrospective study was done including reviewing the clinical data of 12 patients with small cell carcinoma of bladder. All patients with small cell carcinoma of bladder have been successfully followed-up and associated literature was reviewed. Results The small cell carcinoma of bladder was diagnosed in 12 patients including 9 male patients and 3 female patient with a mean age of (64.00±1.24) years old. The stages were T2N0M0 (4 case), T3N0M0 (4 cases), T4N0M0 (3 case) and T4N1M1 (1 case). 2 patients were treated with radical cystectomy after 2 course of chemotherapy, 1 patient was treated with palliative cystectomy after 2 course of chemotherapy, 4 patients were treated with TURBT followed by irrigation of bladder, 2 patients were treated with TURBT only. 2 patients were treated with partial cystectomy, 1 patient was treated with chemotherapy after TURBT. 9 patients died of the disease at an average interval of (10.7±1.92) months, 1 patients died of heart disease about 6 months later, 2 patients have survived for 3 years and 7 years respectively so far. Conclusion The small cell carcinoma of bladder has high malignancy and poor prognosis, it’s hard to cure. Multidrug combination chemotherapy combined with surgery is the main treatment method.
