摘　　要：目的:观察Twist在上皮性卵巢癌组织中的表达情况,分析Twist阳性表达与上皮性卵巢癌患者铂类耐药、无病生存期的关系。方法:回顾性分析2011年至2016年西京医院妇科121例手术患者,对其术后石蜡组织标本进行免疫组化染色,分析Twist在正常卵巢组织、肿瘤组织中的表达,在耐药、敏感组中的表达和相关性分析及其与预后的关系。结果:Twist阳性表达率在正常卵巢组织与肿瘤组织中分别为40.00%、60.40%,其差异有统计学意义(P<0.05)。耐药及敏感组中的上皮性卵巢癌患者中Twist阳性表达率分别为80.00%、52.11%,差异有统计学意义(P<0.05)。上皮性卵巢癌患者中,Twist表达水平越高,患者生存率越低。单因素分析显示:肿瘤复发、FIGO分期、淋巴结转移、铂反应及Twist高表达为影响上皮性卵巢癌患者预后的危险因素;多因素分析显示:复发为上皮性卵巢癌患者的独立预后因素。结论:Twist在上皮性卵巢癌组织中的表达明显高于正常组织,在耐药组的表达明显高于敏感组,但与患者平均无病生存期及总生存期的长短是否也存在统计学差异有待进一步研究。Objective:To observe the expression of Twist in epithelial ovarian cancer tissues,analyze the relationship between Twist positive expression and platinum resistance and disease-free survival in patients with epithelial ovarian cancer.Methods:Retrospective analysis was performed on 121 gynecological patients in Xijing Hospital from 2011 to 2016,who underwent regular postoperative chemotherapy and completed clinical follow-up and prevention.Immunohistochemical staining was performed on paraffin tissue samples after surgery to analyze Twist's expression in normal ovarian tissue,tumor tissue,drug resistance and sensitive tissue and its correlation.Results:The positive expression rate of Twist was 40.00% in normal ovarian tissue and 60.40% in tumor tissue,and the difference was statistically significant( P <0.05).The positive expression rate of Twist in patients with drug resistance and epithelial ovarian cancer in sensitive tissues was 80.00% and 52.11%,respectively,and the difference was statistically significant( P <0.05).The higher the Twist expression level in epithelial ovarian cancer,the lower the survival rate of patients.Single factor analysis showed that recurrence,FIGO stage,lymphatic metastisis,platinum reactin and Twist high expression were influential factors of prognosis of epithelial ovarian cancer patients.Multivariate analysis showed that recurrence was the major factor affecting prognosis of epithelial ovarian cancer patients.Conclusion:The expression of Twist in epithelial ovarian cancer tissue was significantly higher than that in normal tissue,and the expression of Twist in the drug resistance group was significantly higher than that in the sensitive group,and the difference between Twist and the mean disease-free survival time of patients was remain to study further.
