开放手术与腹腔镜手术治疗直径>6 cm肾上腺良性肿瘤临床疗效对比分析

摘　　要：目的对比分析开放手术和腹腔镜手术治疗直径>6cm肾上腺良性肿瘤疗效及安全性差异。方法选取直径>6cm肾上腺良性肿瘤患者共150例,按随机数字表法分为开放手术组(75例)和腹腔镜手术组(75例),分别采用开放手术和腹腔镜手术,比较两组术中术后临床观察指标差异、重症监护病房(ICU)转运率及围手术期并发症发生率。结果腹腔镜手术组术中术后临床观察指标均显著优于开放手术组(P<0.05);两组患者ICU转入率比较差异无统计学意义(P>0.05);腹腔镜手术组术后并发症发生率显著低于开放手术组(P<0.05);两组术中并发症发生率比较差异无统计学意义(P>0.05)。结论相较于开放手术,腹腔镜手术治疗直径>6cm肾上腺良性肿瘤操作用时更短,可实现早期快速恢复,且有助于降低术后并发症发生的风险。Objective To compare the clinical effects and safety of open surgery and laparoscopic surgery in treatment of benign adrenal tumors more than 6 cm in diameter.Methods One hundred and fifty patients with benign adrenal tumors more than 6 cm in diameter were chosen and randomly divided into the open surgery group(n=75,treated with open surgery)and the laparoscopic surgery group(n=75,treated with laparoscopy).The levels of clinical indexes,the ICU transfer incidence and the complication rate were compared the two groups during and after operation.Results The levels of clinical index of the laparoscopic surgery group were better than those of the open surgery group during and after operation(P<0.05).There was no significant difference in the ICU transfer incidence between the two groups(P>0.05).The complication rate of the laparoscopic surgery group was significantly lower than that of the open surgery group after operation(P<0.05).There was no significant difference in the complication rate during operation between the two groups(P>0.05).Conclusion Compared with open surgery,laparoscopic surgery has some advantages in treatment of benign adrenal tumors more than 6 cm in diameter including simple operation,minimal invasion and faster recovery after operation and can efficiently reduce the complication risks.
