摘 要:目的探讨壶腹癌中B7-H1、B7-H4的表达及其与肿瘤浸润T淋巴细胞的关系。方法回顾性总结我院2011年6月-2018年6月外科手术切除的62例壶腹癌标本,将其与18例癌旁正常组织应用免疫组织化学方法检测B7-H1和B7-H4的表达。结果(1)壶腹癌组织中,B7-H1、B7-H4表达的阳性率为80.65%和82.26%,而癌旁正常组织中,B7-H1、B7-H4表达阳性率分别为38.89%和44.44%,两者差异显著(P <0.01);B7-H1和B7-H4表达呈正相关(rs=0.809,P <0.01)。(2)壶腹癌组织中B7-H1、B7-H4阳性标本中CD3+T、CD8+T细胞数量明显低于其阴性的标本。B7-H1阳性表达与CD3+T数量呈负相关(rs=-0.781,P <0.01),CD3+T、CD8+T数量与B7-H4阳性表达呈负相关(rs=-0.752,-0.807;P <0.01)。结论负性协同刺激分子B7-H1和B7-H4在壶腹癌组织中高表达,与T细胞数量有关,参与壶腹癌细胞的免疫逃逸。Objective To investigate the expression of B7-H1 and B7-H4 in Vater’s ampullary carcinoma, and to study the role of B7-H1 and B7-H4 in the regulation of tumor immune surveillance. Methods A retrospective review of 62 cases of ampullary cancer specimens from June 2011 to June 2018 in our hospital, and the expression of B7-H1 and B7-H4 were detected by immunohistochemistry with 18 adjacent normal tissues. Results(1) B7-H1 and B7-H4 positive expression rate was 80.65% and 82.26% in VAC. But in tumor adjacent normal tissue 38.89% of the specimens expressed B7-H1 and 44.44% B7-H4, and statistically significant difference was observed(P < 0.01).B7-H1 expression in VAC was positively correlated with B7-H4 expression(rs=0.809, P < 0.01).(2) The number of CD3+ T and CD8+ T cells in B7-H1 and B7-H4 positive specimens of ampullary carcinoma was significantly lower than that in negative specimens.The positive expression of B7-H1 was negatively correlated with the number of CD3+ T(rs=-0.781, P < 0.01). The number of CD3+ T and CD8+ T was negatively correlated with the positive expression of B7-H4(rs=-0.752,-0.807, P < 0.01). Conclusion Negative costimulatory molecules B7-H1 and B7-H4 is overexpressed in VAC and is correlated with the number T cells and macrophages associated with the tumor. These results suggest that B7-H1 andB7-H4 overexpression play a certain role in the evation of tumor immunity.
