摘 要:目的研究HMBOX1和CD133在胃癌组织中的表达及对预后的影响。方法选取2012年3月-2013年3月山西医科大学第一医院收治的183例胃癌患者为研究对象,均行腹腔镜胃癌根治术,术后随访5年,检测胃癌组织及正常胃黏膜组织中HMBOX1、CD133的表达水平,并分析其与患者预后的关系。结果183例患者胃癌组织中,HMBOX1低表达率为91.80%,对照组的正常表达率为100%;CD133阳性表达率为51.91%,明显高于对照组的14.44%(P<0.05);中位随访时间为42个月,总体生存率为43.17%,HMBOX1表达正常、WHO Ⅱ、Ⅲ、Ⅳ的5年生存率分别为80%、53.62%、40%、23.73%(P<0.05);CD133^+、CD133^-的5年生存率分别为28.42%、59.09%(P<0.05);单因素分析显示,TNM分期、浸润深度、分化程度、淋巴结转移、HMBOX1和CD133表达水平与胃癌患者术后5年生存率显著相关(P<0.05);多因素COX回归分析显示,TNM分期、浸润深度、分化程度、淋巴结转移、HMBOX1和CD133表达水平是影响胃癌患者预后的独立危险因素(P<0.05)。结论HMBOX1在胃癌组织中表达下调,CD133表达上调,两者可作为胃癌患者预后判断的肿瘤标志物。Objective To investigate the expression of HMBOX1 and CD133 in gastric cancer tissues and its impact on prognosis. Methods Included in this study were 183 patients with gastric cancer who were treated at the First Hospital of Shanxi Medical University between March 2012 and March 2013, who underwent laparoscopic radical gastrectomy and were followed up for 5 years. The expression levels of HMBOX1 and CD133 in the gastric cancer tissues and normal gastric mucosa tissues were detected, and their relationship with patient prognosis was analyzed. Results HMBOX1 expression was down-regulated in 91.80% of the 183 patients with gastric cancer, and was normal in 100% of the controls. In the gastric cancer patients, the positive expression rate of CD133 was 51.91%, which was significantly higher than that 14.44% in the control group (P<0.05). The median follow-up length was 42 months, and the overall survival rate was 43.17%. The 5-year survival rates in patients with normal HMBOX1 expression, WHO-Ⅱ,Ⅲ, and Ⅳ classifications were 80%, 53.62%, 40%, and 23.73%, respectively (P<0.05). The 5-year survival rates of CD133+ and CD133- patients were 28.42% and 59.09%, respectively (P<0.05). Univariate analysis showed that TNM stage, depth of invasion, cancer differentiation, lymph node metastasis, HMBOX1 and CD133 expression levels were significantly associated with 5-year survival rates in patients with gastric cancer (P<0.05). Multivariate COX regression analysis showed that TNM stage, depth of invasion, cancer differentiation, lymph node metastasis, HMBOX1 and CD133 expression levels were independent risk factors for the prognosis in patients with gastric cancer (P<0.05). Conclusion In gastric cancer, HMBOX1 is down-regulated and CD133 is up-regulated. The both may be used as tumor biomarkers for predicting the prognosis in gastric cancer patients.
