摘　　要：目的观察阿霉素无水乙醇混合液神经干注射神经损毁治疗三叉神经痛的近期及远期疗效。方法对91例三叉神经痛患者经皮穿刺三叉神经干,注射阿霉素无水乙醇混合液进行神经毁损治疗,记录在治疗前、治疗后当日、7d、1个月、3个月、6个月、1年、2年的疼痛视觉模拟评分(VAS)以及疼痛缓解程度。结果91例患者在1年内的疼痛复发率为5.49%(5/91);治疗后各个时间点的VAS评分均较治疗前明显降低(均P<0.01),治疗后各时间点的VAS评分均明显低于治疗后当日(均P<0.01),治疗后3个月、6个月、1年、2年的VAS评分进行两两比较差异均无统计学意义(均P>0.05);治疗后当日、7d、1个月、3个月、6个月、1年、2年的显效率分别为49.45%、83.52%、95.60%、94.51%、96.55%、93.02%、86.57%;治疗后各时间点的疼痛缓解度平均秩次均明显低于治疗后当日(P<0.05),治疗后1个月、3个月、6个月、1年、2年各时间点的平均秩次均明显低于治疗后7d(均P<0.01),治疗后1个月、3个月、6个月、1年、2年疼痛缓解度的平均秩次进行两两比较差异均无统计学意义(均P>0.05)。所有患者未出现严重并发症。结论阿霉素无水乙醇混合液神经干注射损毁治疗三叉神经痛,操作简便,无严重并发症发生,具有良好的近期及远期疗效。Objectives To observe the short-term and long-term curative effect in the treatm ent of trigem inal neuralgia by nerve trunk injection for nerve damage treatment with m ixture composed of adriamycin and anhydrous ethanol. Methods A total of 91 cases with trigeminal neuralgia patients were treated for nerve damage by transcutaneous puncture and injecting mixture composed of adriamycin and anhydrous ethanol into trigem inal nerve trunk. Pain visual analogue score (VAS) and pain relief level were recorded at the time points of prior treatm ent, the treatm ent day and 7 days, 1 month, 3 months, 6 months, 1 year and 2 years after operation. Results The pain recurrence rate of 91 patients within one year was 5.49%(5/91). VAS score after treatment was significantly lower than that before the treatm ent at each time point and lower than that on the treatm ent day (P < 0.01). The differences were all no statistical significance in VAS score by pairwise comparison between 3 months, 6 months, 1 year and 2 years after treatm ent (P > 0.05). The significant efficiency was 49.45%, 83.52 %, 95.60 %, 94.51 %, 96.55 %, 93.02% and 86.57% respectively on the treatm ent d ay , and 7 days, 1 month, 3 months, 6 months, 1 year, 2 years after treatm ent ( P <0.05). The mean rank of pain relief level at 7 days, 1 month, 3 months, 6 months, 1 year and 2 years after treatm ent were significantly lower than that on the treatm ent day (P <0.05). The m ean rank of each time point at 1 month, 3 months, 6 months, 1 year and 2 years after treatm ent was significantly lower than that at 7 days after treatm ent (P <0.01). There was no statistical significance in the mean rank in pain relief level of 1 month, 3 months, 6 months, 1 year and 2 years after treatm ent by pairwise comparison (P >0.05). No serious com plications occurred in all patients. Conclusions The treatm ent of trigem inal neuralgia by nerve trunk injection for nerve damage treatm ent with mixture composed of adriamycin and anhydrous ethanol is easy to operate and has no serious co
