摘　　要：目的评价虹膜自动跟踪联合Contoura矫正低中度近视伴角膜不规则散光的安全性和有效性。方法前瞻性队列研究。选择2017年1月至8月在我院行近视激光治疗的患者30例(60眼),随机分成两组,其中试验组15例(30眼)接受虹膜自动跟踪定位联合Contoura辅助的LASIK;对照组15例(30眼)接受虹膜自动跟踪定位辅助的LASIK。分别在术前及术后1 d、1个月、3个月观察患者的裸眼视力、最佳矫正视力、表面变异指数(index of surface variance,ISV)、角膜表面垂直不对称指数(index of vertical asymmetry,IVA)、角膜斜轴散光(C3)、角膜规则散光(C5)、对比敏感度等,并作对比。结果术后3个月,试验组裸眼视力为1.12±0.14,高于对照组的1.04±0.19(t=8.58,P<0.05)。术后3个月,试验组ISV和IVA分别降为5.68±3.02和0.89±0.54,与术前比较差异均有统计学意义(均为P<0.05),且均明显低于对照组(均为P<0.05)。术后3个月,试验组C3和C5分别降为(2.10±0.63)D和(2.11±1.04)D,与术前比较差异均有统计学意义(均为P<0.05);对照组C3和C5与术前比较差异均无统计学意义(均为P>0.05)。术后3个月,试验组在明视状态下所有频段均高于对照组(均为P<0.05),在暗视状态下6.0 c·d-1、12 c·d-1时高于对照组(均为P<0.05)。结论虹膜自动跟踪联合Contoura矫正低中度近视伴角膜不规则散光是安全、有效的,能改善角膜的不规则状态,提高视觉质量。Objective To evaluate the safety and efficacy of Contoura Vision with automatic eye tracking system to correct irregular astigmatism and moderate myopia.Methods In this prospective study,30 consecutive patients(60 eyes) with irregular astigmatism and moderate myopia were included from January to August 2017 at Liuzhou Worker’s Hospital.Subjects were randomly divided into an experimental group(15 patients,30 eyes) that underwent Contoura Vision with automatic eye tracking system FS-LASIK and a control group(15 patients,30 eyes) that underwent wavefront-optimized FS-LASIK.The uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),index of surface variance(ISV),and index of vertical asymmetry(IVA),C3,C5 were measured preoperative and at 1 day,1 month and 3 months after operation and the differences between the two groups were compared.Results At 3 months,the postoperative UCVA of experimental group and control group were 1.12±0.14 and 1.04±0.19,respectively,and the difference was statistically significant(t=8.58,P<0.05).Postoperative ISV(5.68±3.02) and IVA(0.89±0.54) were significantly reduced in the experimental group(both P<0.05),and were significantly lower than that in the control group(both P<0.05).At 3 months,C3 and C5 reduced to(2.10±0.63)D and(2.11±1.04)D in the experimental group,and the differences were statistically significant compared with the preoperative(both P<0.05).In the control group,C3 and C5 showed no statistically significant difference compared with the preoperative data(both P>0.05).At postoperative 3 months,the photopic contrast sensitivity at all spatial frequencies in the experimental group were higher than those in the control group(all P<0.05).While the scotopia contrast sensitivity,only 6.0 c·d-1,12 c·d-1 in the experimental group were higher than the control group(both P<0.05).Conclusion Contoura Vision with automatic eye tracking system is safe and effective in astigmatic and myopic correction.Treatment using this technique significantly improved the visual acuit
