合肥市女性人乳头瘤病毒感染亚型分布及其与宫颈病变的相关性

摘　　要：目的探讨合肥地区女性宫颈病变中人乳头瘤病毒(HPV)基因型别及感染模式分布情况。方法回顾性分析2 535例妇科及相关科室就诊并接受HPV分型检测患者的相关临床及病理资料。结果 HPV感染率为29.0%(734/2 535),检出率最高的5种基因型依次为HPV16(7.1%)、HPV52(5.2%)、HPV58(3.9%)、HPV53(1.7%)及HPV33(1.6%)。良性病变/低级别鳞状上皮内病变(LSIL)组中HPV的感染率为50.4%,高级别鳞状上皮内病变(HSIL)/鳞状细胞癌(SCC)组HPV感染率为87.6%,两组感染率差异有统计学意义(χ^2=33.247,P=0.000)。宫颈HSIL+病变中HPV16(37.5%)占比高于HPV58(11.6%)和HPV52(8.9%),两组间差异均有统计学意义(χ^2=3.257、6.917,P <0.05)。结论合肥地区女性HPV优势感染基因亚型分别为HPV16、HPV52、HPV58、HPV53及HPV33。宫颈HSIL+病变中最常见的HPV亚型是HPV16,其次为HPV58和HPV52。Objective To analyze human papillomavirus(HPV)genotype distribution and multi infection modes of cervical lesions in Hefei. Methods Retrospectively we reviewed and analyzed the clinical and pathological data of 2 535 patients,who visited the department of obstetrics and gynecology clinic and received HPV genotyping. Results The overall HPV infection prevalence was 29.0%. HPV16,HPV52,HPV58,HPV53 and HPV33 were the five most popular genotypes with detection rates of 7.1%,5.2%,3.9%,1.7% and 1.6%,respectively. The infection rate of HPV was 50.4% in cervical benign lesions/low-grade squamous intraepithelial lesion(LSIL) and 87.6% in cervical high-grade squamous intraepithelial lesion(HSIL)/squamous cell carcinoma(SCC). The HPV infection rates between the two groups had significant difference(χ^2= 33.247,P = 0.000).The proportion of HPV16(37.5%) in cervical HSIL+ lesions was higher than that of HPV58(11.6%) and HPV52(8.9%). The differences between the two groups were statistically significant(χ^2= 3.257,6.917,P < 0.05).Conclusion The HPV16,HPV52,HPV58,HPV53 and HPV33 are the dominant infection genotypes among women in Hefei. HPV16 is the most common subtype among women with HSIL+ in Hefei city,followed by HPV58 and HPV52.
