取骨钻取自体骨混合同种异体骨在口腔种植中的应用

摘　　要：目的探讨取骨钻钻取自体骨碎屑与同种异体骨移植材料混合移植在口腔种植骨量不足病例中的临床应用效果。方法选取34例上颌前牙区骨量不足的种植患者,分为两组:A组,采用新型取骨钻钻取自体骨碎屑,与同种异体骨移植材料按1∶1比例混合后植入骨缺损区;B组,仅采用同种异体骨移植材料植入骨缺损区。术后随访6个月,通过CBCT对比两组植骨术后牙槽骨宽度骨增量(L1)及术后6个月牙槽骨吸收量(L2)。结果 CBCT评估显示A组术后L1与B组相比差异无统计学意义,但6个月后A组L2小于B组,差异有统计学意义(P <0.05)。结论采用自体骨混合同种异体骨的植骨方式骨移植物吸收率较低,能获得理想的成骨效果,同时新型取骨钻的自体骨取骨方式降低了传统自体骨取骨手术难度,是一种简单实用的方法。Objective To evaluate clinical effectiveness of the mixture of allogeneic bone and autogenous bone which is collected by new-type harvest drillin peri-implant bone augmentation. Methods Clinical data of 34 cases with bone augmentation were analyzed retrospectively,who were divided into two groups:patients in group A were implanted the mixture of allogeneic bone and autogenous bone which is collected by new-type harvest drill,and the proportion of the mixture is 1∶1;Patients in group B were implanted into the bone defect area with allograft materials only. CBCT was used to compare the alveolar bone width increment(L1)and alveolar bone absorption(L2)of the two groups after bone grafting for 6 months. Results CBCT evaluation showed no significant difference between group A and group B in postoperative L1,but after 6 months,L2 of group A was less than that of group B(P < 0.05),indicating a statistically significant difference. Conclusions Bone grafting with autologous bone mixed with allogeneic bone has a low absorptivity and can achieve ideal osteogenic effect. Modified autogenous bone collection by new-type harvest drill is simple and practical,which decreases the complexity of operation and sufferings of patients.
