摘 要:目的:研究微螺钉种植体支抗修复龋齿畸形的美容效果及预后。方法:选择2017年5月-2018年5月在笔者医院口腔科接受治疗的50例龋齿拔除后患者为研究对象。按随机数字表法将所有患者分为观察组与对照组。观察组给予微螺钉支抗种植体正畸法进行治疗,对照组给予口常规外弓强支抗正畸法进行治疗。比较两组患者治疗前后蝶鞍中心、鼻根点以及上齿槽座点构成的角(SNA),蝶鞍中心、鼻根点以及下齿槽座点构成的角(SNB),上齿槽座点、鼻根点以及下齿槽座点构成的角(ANB),上切牙前颅平面角(U1-SN),下切牙下颌平面角(L1-MP),比较两组患者治疗后并发症的发生情况。结果:治疗前,两组患者的SNA、U1-SN数值无显著性差异(P>0.05);治疗后,两组患者的SNA、U1-SN数值均有所降低,且观察组患者的SNA、U1-SN均明显低于对照组,两组数据比较,差异有统计学意义(P<0.05)。治疗前,两组患者的SNB、ANB以及L1-MP数值无显著性差异(P>0.05);治疗后,两组患者的SNB以及L1-MP数值均增加,ANB数值明显降低;且观察组患者的SNB及L1-MP数值均明显高于对照组,观察组的ANB数值明显低于对照组,两组比较差异有统计学意义(P<0.05)。观察组并发症发生率明显低于对照组,两组比较差异有统计学意义(P<0.05)。结论:微型钉种植体支抗用于龋齿拔除后的口腔正畸矫正治疗有较显著疗效,能对患者额面型进行较好塑造,值得临床推广应用。Objective To study the cosmetic effect and prognosis of dental caries repair with micro-screw implant anchorage. Methods From May 2017 to May 2018, 50 patients with dental caries after extraction were selected as the subjects of this study. All patients were divided into observation group and control group according to random number table method. The patients in the observation group were treated with micro-screw anchorage implant orthodontics, while the patients in the control group were treated with conventional external arch anchorage orthodontics. The degrees of SNA, SNB, ANB, U1- SN and L1-MP before and after treatment were compared between the two groups. Compare the complications of two groups after treatment. Results There was no significant difference in SNA and U1-SN values between the two groups before treatment (P >0.05). After treatment, the SNA and U1-SN values of the two groups were decreased, and the SNA and U1- SN values of the observation group were significantly lower than those of the control group (P <0.05). Before treatment, there were no significant differences in SNB, ANB and L1-MP values between the two groups (P >0.05). After treatment, the values of SNB and L1-MP in both groups increased, and the values of ANB decreased significantly. The values of SNB and L1-MP in the observation group were significantly higher than those in the control group, and the values of ANB in the observation group were significantly lower than those in the control group, with significant difference between the two groups (P <0.05). The incidence of complications in the observation group was significantly lower than that in the control group (P <0.05). Conclusion Mini nail implant anchorage is effective in orthodontic treatment after extraction of dental caries. It can better shape the frontal profile of patients and is worthy of wide application.
