桥本甲状腺炎合并甲状腺乳头状癌中环氧化酶-2、Survivin和胸腺基质淋巴细胞的表达及临床意义

摘　　要：目的观察桥本甲状腺炎(Hashimoto’s thyroiditis,HT)合并甲状腺乳头状癌(papillarythyroid carcinoma,PTC)中环氧化酶-2(cyclooxygenase-2,COX-2)、Survivin和胸腺基质淋巴细胞(thymicstromally mphopoietin,TSLP)表达,并分析其临床意义。方法收集2013年10月~2017年10月结节性甲状腺肿40例,HT 80例,PTC 80例,HT合并PTC患者80例。免疫组织化学检测TSLP、Survivin和COX-2的表达,并分析其与HT合并PTC患者临床病理学参数的关系。结果 TSLP在结节性甲状腺肿、HT、PTC以及HT合并PTC中的阳性表达率分别为7.50%、81.25%、88.75%和92.50%,Survivin分别为10.00%、42.50%、77.50%和72.50%,COX-2分别为5.00%、50.00%、90.00%和91.25%。淋巴结转移组TSLP、Survivin以及COX-2阳性表达率显著高于无淋巴结转移组(P<0.05),Ⅲ~Ⅳ期Survivin以及COX-2阳性表达率显著高于Ⅰ~Ⅱ期(P<0.05)。结论 TSLP、Survivin和COX-2异常高表达在肿瘤的发展中起着重要作用。OBJECTIVE To observe the expression of COX-2, Survivin and TSLP in Hashimoto’s thyroiditis combined with thyroid papillary carcinoma and analyze its clinical significance. METHODS From October 2013 to October 2017, 40 cases of nodular goiter, 80 cases of Hashimoto’s thyroiditis(HT), 80 cases of thyroid papillary carcinoma(PTC) and 80 cases of Hashimoto’s thyroiditis combined with thyroid papillary carcinoma(HT combined with PTC) were collected. Immunohistochemistry was used to detect the expressions of TSLP, Survivin and COX-2,and to analyze the relationship between TSLP, Survivin,COX-2 and clinicopathological parameters of HT patients with PTC. RESULTS The positive expression rates of TSLP in nodular goiter, HT, PTC and HT with PTC were 7.50%, 81.25%, 88.75% and 92.50%, respectively.The positive expression rates of Survivin were 10.00%,42.50%, 77.50% and 72.50%, respectively. The positive expression rates of COX-2 were 5.00%, 50.00%, 90.00%and 91.25%, respectively. The positive expression rates of TSLP, Survivin and COX-2 in lymph node metastasis group were significantly higher than those in non-lymph node metastasis group(P <0.05), and the positive expression rates of Survivin and COX-2 in stage Ⅲ-Ⅳ were significantly higher than those in stage Ⅰ-Ⅱ(P <0.05). CONCLUSION High expressions of TSLP, Survivin and COX-2 may play important roles in the development of tumors.
