摘 要:目的探讨 2 型糖尿病患者( T2DM)中血清神经元特异性烯醇化酶( NSE)水平与糖尿病视网膜病变( DR)的相关性。方法选取 400 例 T2DM,分为无 DR( NDR)组 172 例,非增殖期 DR( NPDR)组 126 例,增殖期 DR( PDR)组 102 例,收集空腹血样本用于血脂、血糖、血清 NSE 水平等检测,并对所有 T2DM 者行眼底镜检测。结果 NPDR组、 PDR组患者血清 NSE 水平均高于 NDR组( P < 0. 05),而 NPDR组和 PDR组间差异无统计学意义( P > 0. 05);Logistic回归分析提示,血清 NSE 水平是 DR的独立危险因素( P < 0. 01);血清 NSE 水平预测 DR的ROC 曲线下面积为 0. 803 ( P < 0. 01),当血清 NSE 水平诊断 DR最佳切点值取 11. 78 ng /ml 时,灵敏度为 80. 90%,特异度为 69. 20%。结论血清 NSE 水平是 DR的独立危险因素,在临床上对于血清 NSE 水平> 11. 78 ng /ml 的 T2DM 患者需进一步行眼底镜检查,有助于早期发现及早期治疗 DR患者。Objective To investigate the relationship between serum neuron specific enolase ( NSE) and diabetic retinopathy ( DR) in patients with type 2 diabetes ( T2DM). Methods 400 patients with T2DM were divided into non DR group ( NDR, n = 172),non-proliferative DR ( NPDR,n = 126) group and proliferative DR group ( PDR,n = 102). Their clinical and biochemical parameters were measured. Results Compared with NDR group,patients in NPDR group and PDR group had higher serum NSE level,with the differences statistically significant ( P < 0. 05),but there was no statistical significance on the difference on the NSE level between NPDR group and PDR group ( P > 0. 05 ). According to logistic Logistic regression analysis,serum NSE level was an independent risk factor for DR ( P < 0. 01). Serum NSE levels predict the area under the ROC curve for DR was 0. 803. The optimal cutoff value of serum NSE level for predicting DR was set at 11. 78 ng /ml,its sensi- tivity and specificity were 80. 90% and 69. 20%,respectively. Conclusion Serum NSE level is an independent risk factor for DR,so patients whose serum NSE level is over 11. 78 ng /ml need further test of ophthalmoscope for the earlier diagnose and therapy of DR.
