牙周植骨术+夹板式金属烤瓷联冠在慢性牙周炎伴牙列缺损患者修复中的应用及短期随访研究

摘　　要：目的分析牙周植骨术+夹板式金属烤瓷联冠在慢性牙周炎伴牙列缺损患者修复中的应用效果。方法选取我院慢性牙周炎伴牙列缺损患者157例(2016年3月~2018年4月),依照治疗方式不同分组,对照组78例采取夹板式金属烤瓷联冠治疗,试验组79例采取牙周植骨术+夹板式金属烤瓷联冠治疗,观察对比2组治疗前后探诊深度(PD)、附着丧失(CAL)及红、白美学指数(PES、WES)、治疗后3、6个月牙周炎复发率。结果试验组治疗后CAL、PD值较对照组小(P<0.05);试验组PES、WES评分较对照组高(P<0.05);试验组治疗后3个月牙周炎复发率1.27%较对照组10.26%低(P<0.05),治疗后6个月牙周炎复发率3.80%较对照组14.10%低(P<0.05)。结论慢性牙周炎伴牙列缺损患者采取牙周植骨术+夹板式金属烤瓷联冠治疗,能显著改善其牙周指数及美学指数,且可降低牙周炎复发率。Objective To analyze the effect of periodontal bone grafting and splint metal-ceramic crown in the restoration of chronic periodontitis patients with dentition defect.Methods 157 cases of chronic periodontitis with dentition defect in our hospital (from March 2016-April 2018) were divided into two groups according to different treatment methods.78 cases in the reference group were treated with splint metal-ceramic crowns.79 cases in the experimental group were treated with periodontal bone grafting and splint metal-ceramic crowns.The depth of exploration (PD),loss of attachment (CAL) and red and white aesthetics Index (PES,WES) and recurrence rate of periodontitis 3 and 6 months before and after treatment were observed and compared between the two groups.Results After treatment,the CAL and PD values of the experimental group were lower than that of the control group (P<0.05);the PES and WES scores of the experimental group were higher than than of the control group (P<0.05);the recurrence rate of periodontitis in the experimental group was 1.27% lower than that of the control group (P<0.05);and the recurrence rate of periodontitis in the six months after treatment was 3.80% lower than that of the control group (P<0.05).Conclusion The treatment of chronic periodontitis with dentition defect by periodontal bone grafting and splint metal-ceramic crown can significantly improve the periodontal index and aesthetic index,and reduce the recurrence rate of periodontitis.
