超声乳化白内障吸除术联合人工晶状体植入对青光眼合并白内障患者临床疗效分析

摘　　要：目的探究超声乳化白内障吸除术联合人工晶状体植入治疗青光眼合并白内障的临床效果。方法将2017年1月—2018年7月期间在该院接受诊治的80例青光眼合并白内障患者作为研究对象,并将其随机分至两组,研究组患者采用超声乳化白内障吸除术和人工晶状体植入术为患者进行联合治疗,对照组患者采用小切口非超声乳化白内障摘除术和人工晶状体植入术为患者进行联合治疗,治疗结束后对患者视力情况、手术前后眼压情况进行调查。结果两组患者手术前眼压情况大致相同(40.5±8.7)mmHgvs(40.1±8.3)mmHg,差异有统计学意义(t=0.86,P>0.05);术后1周两组患者眼压(22.8±4.2)mmHgvs(24.9±5.5mmHg)均显著降低,并在术后1个月(23.5±5.0)mmHg,(26.2±6.1)mmHg、术后3个月(23.1±4.8)mmHgvs(26.0±5.8)mmHg和术后6个月(24.3±5.4)mmHg,(27.5±6.6)mmHg逐渐升高,但研究组患者眼压升高幅度更缓慢,数据经检验差异有统计学意义(t=9.73、6.63、5.98、8.56,P<0.05)。结论将超声乳化白内障吸除术和人工晶状体植入术治疗青光眼合并白内障,患者视力大幅提高,眼压改善良好,效果显著,值得推广。Objective To investigate the clinical effect of phacoemulsification and intraocular lens implantation in the treatment of glaucoma with cataract. Methods Eighty patients with glaucoma and cataract who were treated in our hospital from January 2017 to July 2018 were enrolled in the study. They were randomly divided into two groups. The study group was treated with phacoemulsification combined with lens implantation. The patients in the control group underwent treatment of small incision non-phacoemulsification cataract extraction combined with intraocular lens implantation. After the treatment, the visual acuity and intraocular pressure were investigated. Results The intraocular pressure of the two groups was approximately the same (40.5±8.7)mmHg vs (40.1±8.3)mmHg. There was no significant difference between the two groups (t=0.86, P>0.05). The intraocular pressure of the two groups were significantly reduced at 1 week after operation (22.8±4.2)mmHg vs (24.9±5.5)mmHg. At 1 month after surgery (23.5±5.0)mmHg vs (26.2±6.1)mmHg, 3 months after operation (23.1±4.8)mmHg vs (26.0±5.8)mmHg and 6 months after surgery (24.3±5.4)mmHg vs (27.5±6.6)mmHg, the intraocular pressure was gradually increased. But patients in the study group had a slower increase in intraocular pressure, and the data were statistically significant (t=9.73, 6.63, 5.98, 8.56, P<0.05). Conclusion Discussion Phacoemulsification and intraocular lens implantation for glaucoma combined with cataract, could improve the patient's visual acuity significantly, the intraocular pressure. The effect is significant, it is worth promoting.
