常规保乳术与整形保乳术对早期乳腺癌的局部复发及远处转移率的影响

摘　　要：目的探讨常规保乳术与整形保乳术对早期乳腺癌的局部复发及远处转移率的影响。方法回顾性选择85例早期乳腺癌患者,根据手术方法不同,分为观察组(41例)及对照组(44例)。对比2组患者的标本体积、最小及最大手术切缘、术后近期及远期并发症、乳房美容效果客观评分、美容效果、患者满意率、术后1~5年局部复发率及远处转移率。结果观察组患者的切除标本体积,切除标本最小、最大手术切缘,美容效果客观评分,患者满意率明显高于对照组;观察组血肿发生率、血清肿发生率、术后5年局部复发率、远处转移率明显低于对照组(P <0. 05);2组的感染、切口裂开、切口愈合不良、乳头坏死、上臂水肿、远期并发症、术后1~4年局部复发率及远处转移率对比,差异无统计学意义(P> 0. 05)。结论整形保乳术治疗早期乳腺癌有满意的美容效果,且术后长期局部复发率及远处转移率较低。Objective To investigate the conventional breast-conserving surgery and plastic surgery on local recurrence and distant metastasis in patients with early breast cancer. Methods 85 cases with early breast cancer were retrospectively chosen,and were divided into the observation group( 41 cases) and control group( 44 cases) according to different surgery methods.The specimen volume,minimum and maximum surgical margins,postoperative immediate and long-term complications,breast cosmetic effects objective rating,cosmetic results,patient satisfaction rate,local recurrence rate and distant metastasis rate after surgery for 1 ~ 5 years of the 2 groups were compared. Results The resection specimen volume,resection specimen minimum,maximum surgical margin,objective cosmetic score,patient satisfaction rate of the observation group were higher than the control group,the hematoma,incidence of seroma,local recurrence rate after 5 years,distant metastasis rate of the observation group were lower than the control group,P < 0. 05. The infection,incision dehiscence,poor incision healing,papillary necrosis,upper arm edema,long-term complications,local recurrence rate and distant metastasis rate from 1 ~ 4 years after surgery of the 2 groups had no significant difference,P > 0. 05. Conclusion Plastic surgery for breast-conserving surgery has satisfactory cosmetic results for early breast cancer,and lower long-term local recurrence rate and distant metastasis rate.
