摘 要:目的研究HPV多重感染与宫颈癌及癌前病变发生、发展相关性。方法随机选取1451例研究对象,依据病理诊断结果将其分为慢性宫颈炎组(n=229)、鳞状上皮轻度上皮内瘤变组(CINⅠ组,n=644)、鳞状上皮中度上皮内瘤变组(CINⅡ组,n=200)、鳞状上皮重度上皮内瘤变组(CINⅢ组,n=102)和鳞状上皮细胞癌组(SCC组,n=68)、正常组(n=258) 6组,对6组人员的HPV感染情况、HPV感染类型、HPV多重感染类型进行统计分析。结果慢性宫颈炎组、CINⅠ组、CINⅡ组、CINⅢ组、SCC组患者的HPV感染率均显著高于正常组(P <0. 05),CINⅠ组、CINⅡ组、CINⅢ组、SCC组患者的HPV感染率逐渐提升(P <0. 05)。慢性宫颈炎组、CINⅠ组、CINⅡ组、CINⅢ组、SCC组患者的HPV单一型感染率、多重感染率均显著高于正常组(P <0. 05),CINⅠ组、CINⅡ组、CINⅢ组、SCC组患者的HPV单一型感染率逐渐提升(P <0. 05),多重感染率逐渐降低(P <0. 05)。6组人员的HPV二重感染、三重感染、四重感染、五重感染、六重感染比例逐渐降低(P <0. 05),其中CINⅠ组、CINⅡ组、CINⅢ组、SCC组患者的HPV二重感染率显著高于正常组(P <0. 05)。结论 HPV多重感染与癌前病变发生、发展相关,但与宫颈癌无关。Objective To investigate the relationship between HPV multiple infection and occurrence and development of cervical cancer and precancerous lesions. Methods 1451 cases were randomly selected,the patients were divided into chronic cervicitis group( n = 229),squamous intraepithelial neoplasia group( CINⅠ group,n = 644),moderate squamous intraepithelial neoplasia group( CINⅡ group,n = 200),squamous epithelium severe intraepithelial neoplasia group( CINⅢ group,n = 102) and squamous cell carcinoma group( SCC group,n = 68) and normal group( n = 258) 6 groups according to pathological results,the HPV infections,HPV infection types,multiple HPV infection types of the 6 groups were statistically analyzed. Results The HPV infection rates of the chronic cervicitis group,CINⅠ group,CINⅡ group,CINⅢ group and SCC group were significantly higher than the normal group( P < 0. 05),and the infection rates of the CINⅠ group,CINⅡ group,CINⅢ group and SCC group increased gradually( P < 0. 05). the HPV single infection rates of the chronic cervicitis group,CINⅠ group,CINⅡ group,CINⅢgroup,SCC group increased gradually( P < 0. 05),the multiple infection rates gradually decreased( P < 0. 05);The HPV double infection,three infection,four infection,five infection,six infection rates of the 6 groups decreased gradually( P < 0. 05),the HPV double infection rate of the CINⅠ group,CINⅡ group,CINⅢ group,SCC group were significantly higher than the normal group( P < 0. 05). Conclusion Multiple infection of HPV is related to the occurrence and development of precancerous lesions but is not related to cervical cancer.
