帕瑞昔布钠不同时间点超前镇痛对甲状腺癌根治术的应激激素水平的影响

摘　　要：目的探讨帕瑞昔布钠不同时间点超前镇痛对甲状腺癌根治术的应激激素水平的影响。方法选择137例甲状腺癌根治术患者,根据随机数字表法分为A组及B组。A组给予术前30 min帕瑞昔布钠超前镇痛,B组给予帕瑞昔布钠术前30 min及术后12 h超前镇痛。观察2组患者T0~T3点的MAP、HR、Cor、NE及Glu水平,T1~T3点的VAS评分及术后的不良反应。结果 2组T0点的MAP、HR、Cor、NE及Glu水平对比差异无统计学意义(P> 0. 05),A组T1~T3点的MAP、HR、Cor、NE及Glu水平明显高于B组及A组T0点,B组T1、T2点的MAP、HR、Cor、NE及Glu水平均明显高于T0点,P <0. 05。B组T1~T3点的VAS评分均明显低于A组,P <0. 05。2组的不良反应总发生率对比,差异无统计学意义(P> 0. 05)。结论帕瑞昔布钠不同时间点超前镇痛对甲状腺癌根治术患者安全有效,可降低患者手术引起的应激反应。Objective To investigate the influence of parecoxib sodium preanalgesia at different time points on stress hormone level in patients with radical operation of thyroid carcinoma. Methods 137 cases with radical operation of thyroid carcinoma were chosen,who were divided into A group( A group) and B group,A group were given parecoxib sodium preanalgesia before surgery for 30 min,B group were given parecoxib sodium preanalgesia before surgery for 30 min and after surgery for 12 h,the MAP,HR,Cor,NE and Glu level at T0-T3 in the 2 groups were compared,the VAS score at T1-T3 in the 2 groups were compared,the adverse reactions of the 2 groups were compared. Results The MAP,HR,Cor,NE and Glu level at T0 in the 2 groups had no significant difference,P > 0. 05,The MAP,HR,Cor,NE and Glu level at T1-T3 in A group were obvious higher than B group and T0 in A group,the MAP,HR,Cor,NE and Glu level at T1,T2 in B group were obvious higher than T0,P < 0. 05. The VAS score at T1-T3 of the 2 groups in B group were obvious higher than A group,P < 0. 05. The adverse reactions after surgery of the 2 groups had no significant difference,P > 0. 05. Conclusion Parecoxib sodium advanced analgesia at different points is safe and effective in patients with thyroid carcinoma,and which could reduce the intraoperative stress response of patients.
