摘 要:目的利用大鼠饮食模型,探讨基于磁共振成像(MRI)两点法Dixon同相位与反相位技术对非酒精性脂肪肝(NAFLD)及其病变严重程度的诊断价值。方法将30只4周龄的雄性Wister大鼠采用随机数表法随机分为高脂饲养组与正常饲养组,每组15只,正常饲养组进行普通饮食即100%基础饲料饲养,高脂饲养组进行高脂饲养,两组均喂养16周。16周后行腹部轴位Dixon同相位与反相位扫描,分别测量两组肝左、右叶同相位与反相位图像的信号强度并计算高脂饲养组和正常饲养组的肝脂肪分数(HFF)值。扫描结束后,大鼠禁食12 h,行病理学检查,并采用Image ProPlus软件对脂肪变性细胞进行计数,参考Brunt分级标准对非NAFLD进行病理分级(0~3)。采用One-way ANOVA单因素方差分析组间病理分级,采用Pearson相关分析非酒精性脂肪肝病理级与HEF之间的关系。结果高脂饲养组中15只实验鼠为非酒精性脂肪肝,1级2只,脂肪变性细胞约为18%;2级6只,脂肪变性细胞约为60%;3级7只,脂肪变性细胞约为89%。正常饲养组中15只实验鼠均确认无脂肪肝,Brunt分级均为0级。正常饲养组无脂肪肝实验鼠HFF值为(0.03±0.04),高脂饲养组1级、2级、3级脂肪肝实验鼠HFF值分别为(0.15±0.03)、(0.26±0.03)、(0.31±0.02),四组间HFF值比较,差异有统计学意义(F=58.26,P<0.05)。3级脂肪肝实验鼠HFF值显著高于1级(U=2.767)与2级(U=2.058)脂肪肝实验鼠,差异有统计学意义(P<0.05)。HFF与肝细胞中脂变细胞百分比呈现显著的正相关关系(r=0.85,P<0.05)。结论基于MRI两点法Dixon同相位与反相位技术在脂肪含量的诊断中具有较高的应用价值,在对肝脂肪含量的诊断中与病理结果具有较高的一致性。Objective To investigate the diagnostic value of two-point Dixon in-phase and out of phase technique based on magnetic resonance imaging(MRI)in non-alcoholic fatty liver disease(NAFLD)and its severity, a rat diet model was established. Methods Thirty male Wister rats aged 4 weeks were randomly divided into the high-fat group and the normal group by random number table method, 15 rats in each group.The normal group was fed with normal diet, namely 100% basic feed.The high-fat group was fed with high-fat feed, and both groups were fed for 16 weeks.After 16 weeks, the abdominal axial Dixon scanning in the same phase and out of phase was performed, and the signal intensity of the same in phase and out of phase images of the left and right lobe of the liver in the two groups was measured, and the liver fat fraction(HFF)value of the high-fat group and the normal group was calculated.After the scanning, the rats were fasted for 12 hours for pathological examination, and Image ProPlus software was used to count the fatty cells.One-way ANOVA was used to analyze the pathological grading between groups, and Pearson correlation was used to analyze the relationship between the pathological grade of NALFD and HEF. Results In the high-fat group, 15 rats were classified as NAFLD, grade 1 and 2, and steatotic cells were about 18%.6 at grade 2, with about 60% of the steatotic cells;at grade 3, there were 7 steatotic cells, about 89%.All the 15 experimental mice in the normal breeding group were confirmed as free of fatty liver, and Brunt was 0.The HFF values of the experimental rats without fatty liver in the normal feeding group were(0.03±0.04), and those in the high-fat feeding group were(0.15±0.03),(0.26±0.03), and(0.31±0.02), respectively.The difference in HFF values between the four groups was statistically significant(F=58.26, P<0.05). The HFF value of grade 3 fatty liver rats was significantly higher than that of grade 1(U=2.767)and grade 2(U=2.058), and the difference was statistically significant(P<0.05). There was a
