甲胎蛋白阴性原发性肝癌患者C反应蛋白和补体水平及其临床应用价值

摘　　要：目的探讨甲胎蛋白阴性原发性肝癌(PHC)患者血清C反应蛋白(CRP)、补体C3和C4水平及其临床应用价值。方法纳入72例甲胎蛋白阴性PHC患者(甲胎蛋白阴性组)、73例甲胎蛋白阳性PHC患者(甲胎蛋白阳性组)和73例健康志愿者(对照组)。比较3组血清CRP、补体C3及C4水平,并比较不同病理分型PHC患者CRP、C3、C4水平。结果对照组、甲胎蛋白阳性组、甲胎蛋白阴性组血清CRP和C3水平依次升高(P<0.05),且甲胎蛋白阳性组及阴性组C3、C4、CRP阳性率均高于对照组(均P<0.05)。不同病理分型的PHC患者血清CRP、C3、C4水平比较,差异均无统计学意义(P>0.05)。结论相对于正常人及甲胎蛋白阳性的PHC患者,甲胎蛋白阴性PHC者血清CRP和C3水平均异常升高,两者对血清甲胎蛋白阴性肝癌的早期诊断有一定的临床价值。Objective To explore serum C-reactive protein and complements C3 and C4 levels and their clinical utility in alpha-fetoprotein(AFP)-negative patients with primary hepatocellular carcinoma(PHC). Methods Seventy-two AFP-negative PHC patients(AFP-negative group),73 AFP-positive PHC patients(AFP-positive group) and 73 healthy volunteers (control group) were enrolled in this trial.Serum CRP and complements C3 and C4 levels were compared among the three groups,or among PHC patients with different pathological types. Results Serum CRP and C3 levels increased in the order of the control group,the AFP-positive PHC group and the AFP-negative group( P <0.05),and the positive rates of C3,C4 and CRP in the AFP-positive group or the AFP-negative group were significantly higher than those in the control group(all P <0.05).There were no statistically significant differences in serum level of CRP,C3 or C4 among PHC patients with different pathological types ( P >0.05). Conclusion Compared to healthy individuals and AFP-positive patients with PHC,AFP-negative patients with PHC experience an unusual rise in serum CRP and C3 levels,these two indicators have certain clinical value for early-stage diagnosis of serum AFP-negative hepatocellular carcinoma.
