甘精胰岛素联合诺和龙对2型糖尿病患者胰岛功能及TNF-α,IL-6的影响

摘　　要：目的 研究甘精胰岛素联合诺和龙对于高血糖状态的初发2型糖尿病患者胰岛功能、TNF-α及IL-6的作用.方法 选取初发2型糖尿病高血糖状态患者120例,随机平均分为观察组和对照组,每组60例.两组患者均给予糖尿病饮食控制、运动训练和健康知识讲座等基本治疗.对照组在此基础上予以诺和灵30R治疗;观察组在此基础上予以甘精胰岛素联合诺和龙治疗.治疗3个月后复查患者TNF-α、IL-6、胰岛素水平和低血糖事件发生率.结果 观察组患者空腹胰岛素水平显著低于对照组(P<0.05),而餐后2h胰岛素水平显著高于对照组(P<0.05),差异具有统计学意义.观察组患者TNF-α,IL-6水平均显著低于对照组(P<0.05),差异具有统计学意义.低血糖事件发生率观察组显著低于对照组(P<0.05),差异具有统计学意义.结论 甘精胰岛素联合诺和龙控制血糖效果好,能有效改善2型糖尿病高血糖状态患者体内胰岛功能,同时降低TNF-α,IL-6水平.Objective To investigate the effects of insulin glargine combined with Novonorm on islet function,TNF-α and IL-6 in patients with newly diagnosed type2 diabetes mellitus in hyperglycemia.Method 120 patients with newly diagnosed type2 diabetes hyperglycemia were randomly divided into two groups,the observation group and the control group,with 60 cases in each group.The patients of two groups were given basic treatments such as diabetes diet,exercise training and disease knowledge education.On the basis of this,the control group was treated with Novo-Ling 30R;the observation group was treated with insulin glargine combined with Novonorm.After3months of treatment,TNF-α,IL-6,insulin levels and the incidence of hypoglycemia events were reviewed. Results The fasting insulin level in the observation group was significantly lower than that in the control group ( P <0.05),and the insulin level after2 hours the meal was significantly higher than that in the control group ( P <0.05).The difference was statistically significant.The levels of TNF-α and IL-6 in the observation group were significantly lower than those in the control group ( P <0.05),and the difference was statistically significant.The incidence of hypoglycemia in the observation group was significantly lower than that in the control group ( P <0.05),and the difference was statistically significant.ConclusionInsulin glargine combined with Novonorm has good glycemic control effect,can effectively improve islet function in patients with type2 diabetes hyperglycemia,is not prone to hypoglycemia,and simultaneously reduces TNF-α and IL-6 levels,and has a certain effect on preventing insulin resistance and atherosclerosis.
