摘  要:目的探讨达芬奇机器人行原发性腹膜后肿瘤切除术的临床效果。方法 2015年3月~2018年7月我院行达芬奇机器人辅助下原发性腹膜后肿瘤切除术31例,完善术前准备后,全麻下根据肿瘤位置选择手术体位,脐周穿刺建立气腹(压力14 mm Hg),结合术前CT定位,置入trocar。安装达芬奇机器人手术系统,沿肿瘤周围间隙将腹膜后肿瘤完整切除。结果 31例机器人手术均顺利完成。手术时间(从切皮至缝皮完毕) 25~280 min,中位数145 min;术中出血量2~300 ml,中位数10 ml。术后无肠道并发症。术后住院时间(3±1) d。31例随访5~45个月,中位随访时间17个月,均获得无瘤生存,除1例Castleman病和1例弥漫性大B细胞淋巴瘤术后定期行化疗外,其余患者术后均未行放化疗。结论达芬奇机器人手术治疗原发性腹膜后肿瘤安全有效。Objective To investigate the clinical efficacy of the da Vinci robotic surgery for primary retroperitoneal tumor. Methods Between March 2015 and July 2018, 31 patients with primary retroperitoneal tumor received da Vinci robot-assisted surgery in our hospital. After preoperative preparation, under the general anesthesia, the appropriate surgical position was selected according to the location of the tumor. The pneumoperitoneum was established at the umbilical cord (14 mm Hg). Guided with preoperative CT positioning, the trocar was placed. The da Vinci robotic surgical system was installed and the retroperitoneal tumor was completely removed along the peri-tumoral space. Results All the operations were successful. The operation time (from incision cutting to suture) was 25-280 min (median, 145 min). The intraoperative blood loss was 2-300 ml (median, 10 ml). No postoperative intestinal complications occurred. The duration of postoperative hospital stay was (3±1) d. The 31 patients were followed up for 5-45 months (median, 17 months). All the patients obtained tumor-free survival. Except for 1 case of Castleman’s disease and 1 case of diffuse large B-cell lymphoma who received regular postoperative chemotherapy, the other patients were not treated with postoperative radiotherapy or chemotherapy. Conclusion The da Vinci robot-assisted laparoscopic retroperitoneal tumor resection is safe and effective.
