宫瘤清胶囊联合西药治疗子宫肌瘤有效性和安全性Meta分析

摘　　要：目的:系统评价宫瘤清胶囊联合西药治疗子宫肌瘤的有效性和安全性。方法:计算机检索the Cochrane Library (2019年2期)、PubMed、Embase、中国知网(CNKI)、中国生物医学文献数据库(CBM)、维普和万方数据库,搜集宫瘤清胶囊联合西药与单纯西药比较治疗子宫肌瘤的随机对照试验研究论文,检索时限均从建库至2019年2月。按照纳入、排除标准筛选文献、提取资料,对子宫肌瘤、子宫体积、临床总有效率、雌二醇(E2)、孕酮(P)及不良反应进行Meta分析。结果:共纳入19个随机对照试验研究论文,对其中17篇试验组采用宫瘤清胶囊联合米非司酮与对照组单纯采用米非司酮的研究进行Meta分析,结果显示,联合用药组子宫肌瘤体积比单纯西药组缩小更明显[MD=3.78,95%CI(2.22,5.35),P<0.000 01],治疗前后子宫体积差大于单纯西药组[MD=17.13,95%CI(11.82,22.44),P <0.000 01],临床总有效率高于单纯西药组[RR=1.22,95%CI(1.16,1.28),P <0.000 01],E2、P水平均低于对照组,E2:[MD=-26.68,95%CI (-36.20,-17.16),P <0.000 01];P:[MD=-0.74,95%CI(-1.14,-0.35),P <0.000 01]。联合用药组和单纯西药组治疗过程中均未见明显不良反应发生。结论:宫瘤清胶囊联合西药较单纯使用西药治疗子宫肌瘤效果更好,可有效缩小子宫肌瘤,降低雌孕激素水平,但受纳入研究质量限制,此结论需开展更多高质量研究予以验证。Objective: To evaluate the efficacy and safety of Gongliuqing capsules combined with western medicine for hysteromyoma systematically. Methods: Applied the computer to retrieve The Cochrane Library(2019, 2), PubMed,Embase,China Knowledge Network(CNKI),China Biomedical Literature Database(CBM),VIP and Wanfang database,and to search a randomized controlled trial study on the comparative treatment of Gongliuqing capsules combined with western medicine for hysteromyoma and that of western medicine alone,in which these studies were selected from the establishment of databases to February 2019. Extracted researches and selected data according to the inclusion and exclusion criteria,and performed Meta analysis on hysteromyoma, uterine volume, the total clinical effect, estrogen(E2), progesterone(P) and adverse reactions. Results: A total of 19 randomized controlled trials were included, in which 17 of them about the application of Gongliuqing capsules combined with mifepristone in the experiment group and the application of western medicine of mifepristone in the control group were given Meta analysis. The results showed that the tumor volume reduction of hysteromyoma of the combined drug application group was clearer than that of the western medicine group [MD=3.78,95% CI(2.22,5.35),P < 0.000 01];the difference in uterine volume before and after treatment was greater than that in the western medicine group [MD=17.13,95% CI(11.82,22.44),P<0.000 01];the total clinical effective rate was higher than that of the western medicine group [RR=1.22,95% CI(1.16,1.28),P<0.000 01];and the E2 and P levels were lower than those in the western medicine group,E2:[MD =-26.68,95% CI(-36.20,-17.16),P<0.000 01];P:[MD =-0.74,95%CI(-1.14,-0.35), P<0.000 01]. No obvious adverse reactions were found during the treatment in the combined drug application group and the western medicine group. Conclusion: The application of Gongliuqing capsules combined with western medicine has better effect in treating hysteromyoma than that of the application
