摘  要:目的:观察醒脑静注射液联合吞咽功能训练治疗缺血性脑卒中吞咽障碍的临床疗效及对血清可溶性白细胞介素-2受体(SIL-2R)和肿瘤坏死因子(TNF)-α水平的影响。方法:将缺血性脑卒中吞咽功能障碍患者120例随机分为对照组和治疗组,各60例;对照组予吞咽功能训练,治疗组在对照组的基础上予醒脑静注射液静脉滴注,2组疗程均为4周;观察比较2组洼田饮水试验、吞咽困难分级及标准吞咽功能评价量表(SSA)评分和临床疗效,检测2组血清SIL-2R和TNF-α水平。结果:总有效率治疗组为91.67%,对照组为76.67%,2组比较,差异有统计学意义(P <0.05)。治疗后,2组洼田饮水试验、吞咽困难分级、SSA评分均较治疗前明显降低(P <0.05);且治疗组上述各项评分均低于对照组(P <0.05)。治疗后,2组SIL-2R、TNF-α水平均较治疗前明显降低(P <0.05);且治疗组上述两项指标均低于对照组(P <0.05)。结论:醒脑静注射液联合吞咽功能训练治疗缺血性脑卒中吞咽障碍的疗效肯定,其调节SIL-2R、TNF-α水平可能是其疗效途径之一。Objective:To observe the clinical effect of Xingnaojing injection combined with swallowing function training for dysphagia in ischemic stroke and its effect on the levels of serum soluble interleukin-2 receptor(SIL-2 R) and tumor necrosis factor(TNF)-α. Methods:A total of 120 cases of patients with dysphagia in ischemic stroke were randomly divided into the control group and the treatment group, 60 cases in each group. The control group was treated with swallowing function training,and the treatment group was additionally given Xingnaojing injection by intravenous drip based on the treatment of the control group. Both groups were treated for four weeks. The scores of water swallow test, grading of dysphagia and standardized swallowing assessment(SSA) as well as the curative effect in the two groups were observed and compared,and the levels of serum SIL-2 R and TNF-α in the two groups were detected. Results:The total effective rate was 91.67% in the treatment group and 76.67% in the control group,the difference being significant(P < 0.05). After treatment,the scores of water swallow test,grading of dysphagia and SSA in the two groups were obviously decreased when compared with those before treatment(P < 0.05);and the above scores in the treatment group were lower than those in the control group(P <0.05). After treatment,the levels of SIL-2 R and TNF-α in the two groups were obviously decreased when compared with those before treatment(P < 0.05);and the above levels in the treatment group were lower than those in the control group(P < 0.05). Conclusion:The therapy of Xingnaojing injection combined with swallowing function training has significant curative effect in treating dysphagia in ischemic stroke, whose mechanism is probably the regulation of the levels of SIL-2 R and TNF-α.
