血清CEA、CA125、CYFRA21-1水平与肺腺癌T NM分期及治疗方法的相关性分析

摘　　要：目的探析血清CEA、CA125、CYFRA21-1水平与肺腺癌患者TNM分期及治疗方法选择的相关性。方法选择2017年10月—2018年10月该院收治的200例肺腺癌患者为研究对象,收集患者的TNM分期以及治疗方法等临床资料,并在其接受治疗前进行血清CEA、CA125、CYFRA21-1检测,分析各项指标与TNM分期、治疗方法的关系。结果Ⅲa期、Ⅲb期、Ⅳ期的血清CEA、CA125、CYFRA21-1检测值均逐渐升高,差异有统计学意义(F=764.169,P=0.000;F=110.692,P=0.000;F=483.813,P=0.000);保守治疗者的CEA、CA125、CYFRA21-1值显著高于局部切除术(t=21.644,P=0.000;t=9.160,P=0.000;t=15.388,P=0.000);而局部切除术的CEA、CA125、CYFRA21-1检测值也较根治性手术显著升高(t=47.980,P=0.000;t=18.226,P=0.000;t=23.748,P=0.000)。结论血清CEA、CA125、CYFRA21-1水平高低与肺腺癌患者的TNM分期、治疗方法选择有密切的关系,故临床可将血清CEA、CA125、CYFRA21-1检测作为评估肺腺癌病情进展的主要辅助指标,有助于临床制订肺腺癌治疗方案以及判断患者的预后。Objective To analyze the correlation between serum CEA,CA125,CYFRA21-1 levels and TNM staging and treatment options in patients with lung adenocarcinoma.Methods A total of 200 patients with lung adenocarcinoma admitted to our hospital from October 2017 to October 2018 were enrolled.The clinical data of TNM staging and treatment methods were collected and serum CEA,CA125,CYFRA21-1 detection were performed before treatment,analysis of the relationship between various indicators and TNM staging,treatment methods.Results The serum levels of CEA,CA125 and CYFRA21-1 in stage IIIa,IIIb and IV were gradually increased,and the difference was statistically significant(F=764.169,P=0.000;F=110.692,P=0.000;F=483.813,P=0.000);CEA,CA125,CYFRA21-1 values of conservative treatment patients were significantly higher than local resection(t=21.644,P=0.000;t=9.160,P=0.000;t=15.388,P=0.000);The CEA,CA125,and CYFRA21-1 values of local excision were also significantly higher than those of radical surgery(t=47.980,P=0.000;t=18.226,P=0.000;t=23.748,P=0.000).Conclusion The levels of serum CEA,CA125 and CYFRA21-1 are closely related to the TNM staging and treatment options of patients with lung adenocarcinoma.Therefore,serum CEA,CA125 and CYFRA21-1 can be used as the main factors to evaluate the progress of lung adenocarcinoma auxiliary indicators help to develop clinical treatment plans for lung adenocarcinoma and to judge the prognosis of patients.
