摘 要:目的 探讨miR-143、miR-145联合血清甲胎蛋白(alpha-fetoprotein,AFP)对原发性肝癌的临床诊断价值。方法 选取2016年7月—2017年9月我院收治的原发性肝癌60例作为肝癌组,取同期肝硬化60例作为肝硬化组、健康体检者60例作为健康对照组。比较3组miR-143、miR-145、AFP水平,计算不同检查方式诊断原发性肝癌的灵敏度及特异度。结果 肝癌组miR-143、miR-145、血清AFP水平高于肝硬化组和健康对照组(P<0.05)。miR-143+miR-145+AFP诊断原发性肝癌的灵敏度及特异度高于AFP、miR-145+AFP、miR-143+AFP单一诊断(P<0.05)。结论 miR-143、miR-145在原发性肝癌患者血清中表达水平升高,联合AFP检测可提高诊断原发性肝癌的灵敏度及特异度。Objective To investigate clinical value of miR- 143 and miR-145 combined with serum alpha-fetoprotein (AFP) in diagnosis of patients with primary liver cancer.Methods A total of 60 patients with primary liver cancer admitted during July2016 and September2017 were selected as liver cancer group, and another 60 patients with liver cirrhosis at the same period were selected as liver cirrhosis group, and 60 healthy people taking physical examination were selected as control group.Among three groups, levels of miR-143, miR-145 and AFP were compared, and sensitivity and specificity values of different examination methods in diagnosis of patients with primary liver cancer were calculated respectively.Results Levels of miR-143, miR-145 and AFP in liver cancer group were significantly higher than those in liver cirrhosis and healthy control groups ( P <0.05).Sensitivity and specificity values of miR-143 + miR-145 +AFP in diagnosis of patients with primary liver cancer were significantly higher than those by AFP, miR-145 +AFP and miR-143 +AFP alone ( P <0.05). Conclusion Expressions of miR-143 and miR-145 in serum of patients with primary liver cancer are increased, and combined detection of AFP may improve sensitivity and specificity in diagnosis of patients with primary liver cancer.
