腹腔镜下系统保留盆腔自主神经的宫颈癌根治术效果评价

摘　　要：目的:评价腹腔镜下系统保留盆腔自主神经的宫颈癌根治术效果。方法:宫颈癌手术患者60例,随机分为两组,对照组应用传统宫颈癌根治术治疗,研究组应用保留盆腔自主神经的宫颈癌根治术治疗。结果:在手术指标对比方面,两组患者手术时间、术中出血量比较有明显差异(P<0.05),术中中转开腹例数和术中/术后并发症发生率等对比无明显差异(P>0.05)。在膀胱功能以及直肠功能对比方面,研究组患者的术后残余尿时间以及排气、排便时间均显著短于对照组(P<0.05);尿流动力学指标对比方面,两组患者术前膀胱灌注阶段以及排尿阶段的各项指标对比无明显差异(P>0.05),术后研究组各项指标显著优于对照组(P<0.05);两组患者术前生活质量以及性生活质量对比无明显差异(P>0.05),术后研究组各项指标显著优于对照组(P<0.05)。结论:宫颈癌根治术患者通过系统保留盆腔自主神经,能够改善患者的膀胱功能、直肠功能以及尿动力学指标。Objective:To investigate the effect of pelvic autonomic nerve sparing surgery in the treatment of cervical cancer. Methods:60 cases of radical prostatectomy were selected in our hospital from 2012 to2015 cervical cancer were randomly divided into two groups.The control group used the traditional radical hysterectomy of cervical cancer,and the study group used nerve sparing radical surgery for cervical cancer. Results:There was obvious difference between the two groups of patients with operation time and hemorrhage during operation ( P <0.05).Cases of conversion to laparotomy and the rate of complication had no significant difference ( P >0.05).In the bladder function and rectal function comparison,the study group of patients with postoperative residual urine time and exhaust time,defecation time were significantly shorter than the control group ( P <0.05).In urodynamic index comparison,no significant difference between the index contrast of two groups of patients for intravesical stage and voiding phase before operation ( P >0.05).After operation,the research group was better than the control group ( P <0.05). Conclusion:Pelvic autonomic nerve preservation through the system radical resection of cervical cancer,can improve the patient's bladder,rectum function and urodynamic index.
