摘 要:目的 总结肿瘤型膝关节置换术后的早期康复治疗方法,观察抗阻运动训练对患者膝关节功能恢复的影响。方法 收集2016年10月至2018年9月我院收治的接受肿瘤型膝关节置换的股骨远端肿瘤患者46例,根据术后康复治疗的不同,将患者分为治疗组与对照组,每组23例。对照组采用常规康复治疗,治疗组采用抗阻运动训练和常规康复治疗。结果 治疗组与对照组治疗前膝关节活动屈曲活动度、膝关节活动伸直活动度、膝关节疼痛评分、膝关节功能评分比较差异均无统计学意义(P均>0.05)。2组治疗后膝关节活动屈曲活动度、膝关节活动伸直活动度比较差异均有统计学意义(P均<0.05);膝关节疼痛和功能评分比较差异均有统计学意义(P均<0.05);生理健康、心理健康、社会关系和环境评分等生活质量评分比较差异均有统计学意义(P均<0.05)。结论 抗阻运动训练能够改善肿瘤型膝关节假体置换术后患者早期膝关节屈伸活动度,减轻疼痛,促进膝关节功能恢复,并提高生活质量方面。Objective To summarize the early rehabilitation treatment after tumor knee prosthesis replacement, and to observe the effect of resistance exercise training on knee function recovery. Methods Forty-six patients with distal femur tumor treated with tumor knee prosthesis replacement were divided into the observation group and the control group, 23 cases in each group according to different treatment methods. The 23 patients in the control group were treated with conventional treatment, while 23 patients in the observation group were treated with resistance exercise training and conventional treatment. Results There was no significant difference in the preoperative knee flexion degree, knee extension degree, knee joint pain score and knee joint function score between the observation group and the control group ( P >0.05). There were significant differences in the postoperative knee flexion degree and knee extension degree between the two groups ( P <0.05). There were significant differences in the postoperative knee joint pain and function scores between the two groups ( P < 0.05), and there were significant differences in the postoperative quality of life (physical health,mental health,social relations,environment) between the two groups ( P <0.05). Conclusion Resistance exercise training can improve the flexion and extension of knee joint, relieve pain and promote the recovery of knee joint function after tumor knee prosthesis replacement, it has prominent value in improving the quality of life.
