摘 要:目的探讨不同疾病来源幽门螺杆菌(H.pylori)对蒙古沙土鼠(MGs)胃黏膜损伤的影响。方法将30只雄性SPF级MGs随机分为对照组、胃炎分离H.pylori组(胃炎株组)和胃癌分离H.pylori组(胃癌株组),每组各10只。各组MGs经灌胃给予不同来源的H.pylori,每日1次,连续3d。灌胃后第26周处死动物,组织病理学评价不同菌株对MGs胃黏膜的损伤,免疫印迹试验检测胃黏膜组织核因子-红系2相关因子2(Nrf2)、Nqo1和Ho-1蛋白表达。结果胃癌株组80%胃黏膜发生糜烂、溃疡,溃疡直径较大,边缘不整,伴发黏膜出血;胃炎株组40%胃黏膜发生黏膜糜烂、溃疡,溃疡直径较小。镜下可见,胃癌株组发生明显的炎症细胞浸润、黏膜糜烂、溃疡和肠黏膜化生等病理改变,而胃炎株组黏膜损伤程度较轻,仅见少量炎症细胞浸润。与对照组比较,胃炎株组和胃癌株组黏膜Nrf2蛋白表达增加,Nqo1和Ho-1蛋白表达降低,差异有统计学意义(P<0.05);与胃炎株组比较,胃癌株组Nrf2蛋白表达增加,Nqo1和Ho-1蛋白表达降低,差异有统计学意义(P<0.05)。结论不同疾病来源的H.pylori对MGs胃黏膜的损伤明显不同,胃癌来源的H.pylori对MGs胃黏膜的损伤能力更强,这种损伤差异可能与Nrf2/Ho-1通路的激活相关。Objective To investigate the effects of Helicobacter pylori(H.pylori) from different sources on gastric mucosal lesions in Mongolian gerbils(MGs). Methods Thirty male SPF MGs were randomly divided into control group,gastritis isolation H.pylori group(gastritis strain group)and gastric cancer isolation H.pylori group(gastric cancer strain group),with 10 in each group.H.pylori from different sources was given orally once a day for 3 consecutive days.Animals were sacrificed 26 weeks after intragastric administration.Histopathology was used to evaluate the gastric mucosal damage caused by different strains of MGs.The expression of nuclear factor-erythroid 2(Nrf2),Nqo1 and Ho-1 in gastric mucosa was detected by Western blot. Results Generally,80% of gastric mucosa in gastric cancer strain group suffered from erosion and ulcer.The diameter of ulcer was larger,the margin was irregular,accompanied by mucosal hemorrhage,while 40% of gastritis strain group suffered from mucosal erosion and ulcer,and the diameter of ulcer was smaller.Microscopically,obvious inflammatory cell infiltration,mucosal erosion and ulcer,intestinal metaplasia and other pathological changes were observed in gastric cancer strain group,while mucosal damage in gastritis strain group was milder,only a small amount of inflammatory cell infiltration was observed,and no obvious mucosal atrophy was observed.Compared with the control group,the expression of Nrf2 protein increased and the expression of Nqo1 and Ho-1 protein decreased in gastritis group and gastric cancer group,with statistically significant difference( P <0.05).Compared with gastritis group,the expression of Nrf2 protein increased and the expression of Nqo1 and Ho-1 protein reduced in gastric cancer group,with statistically significant difference( P <0.05). Conclusion H.pylori from different sources of disease has different damage to gastric mucosa of MGs.H.pylori from gastric cancer has stronger damage to gastric mucosa of MGs.This difference may be related to the activation of Nrf2/Ho-1 pa
