牙髓血运重建术治疗年轻恒牙牙髓和根尖周病变的疗效观察

摘　　要：目的:观察牙髓血运重建术治疗年轻恒牙牙髓和根尖周病变的临床效果。方法:选择牙髓和根尖周病变的单根管年轻恒牙10例,行牙髓血运重建术。术后定期行临床和X线片检查,观察牙髓和根尖周病变的愈合及牙根发育情况。结果:牙髓血运重建术后进行12~24个月的随访,10例患者无不适临床症状和体征,9例牙根继续发育,牙根长度和根管壁厚度均有不同程度增加;1例嵌入性牙脱位致牙髓坏死患牙术后15个月无临床症状和阳性体征,牙根长度增加及根尖聚拢均不明显,但根管壁有明显不规则增厚;10例牙髓电活力测试均无阳性反应。结论:牙髓血运重建术治疗牙髓和根尖周病变的年轻恒牙可使患牙获得与正常发育牙齿相似的牙根长度和根管壁厚度,但长远效果有待长期观察随访。Objective:To evaluate the clinical effect of pulp revascularization in immature permanent teeth with pulpitis and apical periodontitis.Methods:Ten immature permanent teeth diagnosed with pulpitis or apical periodontitis were selected and received pulp revascularization procedures.These teeth were recalled periodically after the treatment.The clinical effect was evaluated through clinical and radiographic examination.Results:12 to 24 months after treatment,10 teeth showed normal clinical and radiographic manifestation,9 of the 10 teeth showed root length and dentin wall thickness increase.One tooth intrusive luxation showed dentin wall thickness increase,but increase in root length and apical foramen closure were not observed.The electric pulp vitality test was positive in 10 teeth.Conclusion:Pulp revascularization in young permanent teeth with pulpitis and apical periodontitis resulted in similar clinical outcomes in root development and root canal wall formation compared with the contralateral control teeth.However,the long-term effect remains to be followed up for a long time.
