摘  要:目的:探讨小细胞肺癌患者发生骨转移及骨相关事件的风险因素。方法:回顾研究2012年10月至2013年4月第三军医大学新桥医院110例广泛期小细胞肺癌患者的临床资料,单因素分析用单因素方差分析模型,骨转移风险因素的多因素分析用COX比例风险模型。发生骨相关事件的因素分析用二项分类Logistic回归分析。结果:110例小细胞肺癌患者中,发生骨转移42例,骨转移发生率为38.18%。单因素和多因素分析示性别、年龄、吸烟、其他部位转移不是小细胞肺癌骨转移的风险因素(P>0.05)。42例骨转移患者中18例(42.86%)发生骨相关事件,其中有4例发生1次以上骨相关事件。多发骨转移灶是小细胞肺癌骨转移患者发生骨相关事件的风险因素(P<0.05)。结论:小细胞肺癌骨转移患者中多发骨转移灶是其发生骨相关事件的风险因素,要对多发骨转移灶患者加强观察和保护,并尽早积极治疗。Objective:To identify the risk factors of bone metastases and skeletal related events(SREs)in small cell lung cancer(SCLC)patients.Methods:The clinical data of 110 SCLC patients at Xinqiao Hospital of Third Military Medical University between October 2012 and April 2013 were retrospectively analyzed.Make use of single effect analysis of variance model to deal with all single effect analyzed data.The risk factors were analyzed by COX proportional hazard model for bone metastases and by binary Logistic regression for SREs.Results:Among 110 SCLC patients,42 patients(38.18%)had bone metastases.Gender,age,smoking and other organ metastasis were not risk factors for bone metastases.In 42 patients with bone metastases,18 case(42.86%)had SREs,in which SREs of 4 cases were more than 1 time.Multi-bone metastases were more likely to have SREs.Conclusion:Multi-bone metastases were more likely to have SREs in SCLC patients with bone metasrases,so more attention and protection should be paid to these patients and active treatment should be delivered to them as early as possible.
