摘 要:目的探讨液基薄层细胞检查(TCT)联合人乳头状瘤病毒DNA分型检测在宫颈癌筛查中的应用价值。方法纳入2017年6~12月在我院行宫颈癌筛查的130例患者,收集其TCT及HPV-DNA检测标本,取活检标本进行病理检查,以病理检查结果为金标准,分析TCT、HPV-DNA及两者联合在判断宫颈癌病变中的诊断效能。结果以病理检查为金标准,TCT检测符合率为61.54%(80/130),HPV-DNA检测符合率为69.23%(90/130),差异无统计学意义(χ2=1.699,P=0.192);随着TCT级别升高,高危型HPV表达率逐渐升高,在SCC中高危型HPV表达率为100%,差异有统计学意义(P<0.05);TCT联合HPV-DNA在诊断灵敏度、特异度、准确度方面均高于单一TCT、HPV-DNA检测,差异有统计学意义(P<0.05)。结论TCT联合HPV-DNA分型检测能够提高早期宫颈癌筛查准确率,在临床上具有较高应用价值。Objective To explore the application values of thin-layer cytological test (TCT) combined with human papillomavirus DNA genotyping test in cervical cancer screening. Methods 130 patients who underwent cervical cancer screening in our hospital from June 2017 to December 2017 were selected.TCT and HPV-DNA test specimens were collected,and biopsy specimens were taken for pathological examination.The pathological examination results were used as the gold standard,and the diagnostic efficacy of TCT,HPV-DNA and the combination of TCT and HPVDNA in judging cervical cancer lesions was analyzed. Results With pathological examination as the gold standard,the coincidence rate of TCT detection was 61.54%(80/130)and the coincidence rate of HPV-DNA detection was 69.23%(90/130).There was no statistically significant difference (χ2=1.699,P=0.192).As the TCT level increased,the expression rate of high-risk HPV gradually was increasedand the expression rate of high-risk HPV in SCC reached 100%.The difference was statistically significant (P < 0.05).The diagnostic sensitivity,specificity and accuracy of TCT combined with HPV-DNAwere higher than that of single TCT and HPV-DNA detection.The difference was statistically significant (P < 0.05). Conclusion TCT combined with HPV-DNA genotypingcan improve the accuracy of early cervical cancer screening,which has higher clinical application value.
